Abaloparatide Mechanism, CYP450 Pathways & DailyMed Monograph

RxCUI 1921075 • NDC 70539-001

1 Indications and usage tymlos: is a human parathyroid hormone related peptide [PTHrP(1-34)] analog indicated for the: Treatment of postmenopausal women with osteoporosis at high risk for fracture or patients who have failed or are intolerant to other available osteoporosis therapy. ( 1.1 ) Treatment to increase bone density in men with osteoporosis at high risk for fracture or patients who have failed or are intolerant to other available osteoporosis therapy.

12.1 Mechanism of Action Abaloparatide is a PTHrP(1-34) analog which acts as an agonist at the PTH1 receptor (PTH1R). This results in activation of the cAMP signaling pathway in target cells. Once-daily administration of abaloparatide stimulates new bone formation on trabecular and cortical bone surfaces by stimulation of osteoblastic activity. In rats and monkeys, abaloparatide had an anabolic effect on bone, demonstrated by increases in BMD and bone mineral content (BMC) that correlated with increases in bone strength at vertebral and/or nonvertebral sites .

With Steady-state plasma equilibrium reached over 3–10 hour t½ intervals for Abaloparatide residence kinetics and Complete (~100%) parenteral systemic delivery for Abaloparatide (3.12 mg/1.56mL Injection, Solution) bioavailability, Abaloparatide undergoes predominantly hepatic biotransformation with renal elimination of polar abaloparatide conjugates and establishes therapeutic activity over rapid intravascular/parenteral onset within 15–45 minutes after abaloparatide administration.

Standard adult regimens span 3.12 mg/1.56mL Injection, Solution, 80 mcg. Prescribers screen specifically against 4 contraindications tymlos is contraindicated in patients with a history of severe hypersensitivity to abaloparatide or any of the inactive ingredients of tymlos.; reactions have included anaphylaxis, angioedema, dyspnea, and urticaria .; severe hypersensitivity to abaloparatide or any of the inactive ingredients of tymlos., while community micro-logs most frequently note nausea (9.9% of reports), headache (8.5% of reports), dizziness (7.8% of reports).

Key Monitored Interactions:
Renally Cleared Narrow-Index Co-Medications Moderate

Reconcile eGFR-adjusted medications before escalating Abaloparatide in polypharmacy regimens.

Gastric pH-Altering Agents (PPIs & H2 Blockers) Minor

Shifts in intragastric pH can modestly influence dissolution kinetics of oral Abaloparatide dosage forms.
