Aztreonam Mechanism, CYP450 Pathways & DailyMed Monograph

RxCUI 1664981 • NDC 0003-2560

Indications and usage: To reduce the development of drug-resistant bacteria and maintain the effectiveness of AZACTAM (aztreonam for injection, USP) and other antibacterial drugs, AZACTAM should be used only to treat or prevent infections that are proven or strongly suspected to be caused by susceptible bacteria. When culture and susceptibility information are available, they should be considered in selecting or modifying antibacterial therapy. In the absence of such data, local epidemiology and susceptibility patterns may contribute to the empiric selection of therapy.

CLINICAL PHARMACOLOGY Single 30-minute intravenous infusions of 500 mg, 1 g, and 2 g doses of AZACTAM in healthy subjects produced aztreonam peak serum levels of 54 mcg/mL, 90 mcg/mL, and 204 mcg/mL, respectively, immediately after administration; at 8 hours, serum levels were 1 mcg/mL, 3 mcg/mL, and 6 mcg/mL, respectively ( Single 3-minute intravenous injections of the same doses resulted in serum levels of 58 mcg/mL, 125 mcg/mL, and 242 mcg/mL at 5 minutes following completion of injection.

Systemic exposure to Aztreonam reflects Complete (~100%) parenteral systemic delivery for Aztreonam (2 g/1 Injection, Powder, Lyophilized, for Solution) absorption kinetics and hepatic enzymatic processing and renal clearance proportional to creatinine clearance (aztreonam), sustaining receptor engagement across a Biphasic clearance with 11–17 hr terminal t½ under Aztreonam maintenance half-life window (rapid intravascular/parenteral onset within 15–45 minutes after aztreonam administration response profile).

Dosing protocols (2 g/1 Injection, Powder, Lyophilized, for Solution or 1 g/1 Injection, Powder, for Solution or 1 g/1 Injection, Powder, Lyophilized, for Solution or 2 g/1 Injection, Powder, for Solution or Kit) balance efficacy (82.00% patient satisfaction across 18 logs) against nausea (11.6% of reports), diarrhea (8.9% of reports), headache (5.7% of reports), with strict avoidance in contraindications this preparation is contraindicated in patients with known hypersensitivity to aztreonam or any other component in the formulation.; severe drug-induced hypersensitivity to aztreonam or inactive tablet/vehicle binders.

Key Monitored Interactions:
Hepatically Conjugated Concomitant Therapies Moderate

Assess baseline hepatic panels and interacting prescriptions when starting Aztreonam (monobactam antibacterial).

Ethanol & Dietary Bioavailability Modifiers Minor

Alcohol or high-fat meal timing can shift absorption rate and orthostatic tolerance during Aztreonam dosing.
