Carbidopa Mechanism, CYP450 Pathways & DailyMed Monograph

RxCUI 260260 • NDC 16714-067

Indications and usage: Carbidopa tablets are indicated for use with carbidopa-levodopa or with levodopa in the treatment of the symptoms of idiopathic Parkinson’s disease (paralysis agitans), postencephalitic parkinsonism, and symptomatic parkinsonism, which may follow injury to the nervous system by carbon monoxide intoxication and/or manganese intoxication. Carbidopa tablets are for use with carbidopa-levodopa in patients for whom the dosage of carbidopa-levodopa provides less than adequate daily dosage (usually 70 mg daily) of carbidopa.

CLINICAL PHARMACOLOGY Parkinson’s disease is a progressive, neurodegenerative disorder of the extrapyramidal nervous system affecting the mobility and control of the skeletal muscular system. Its characteristic features include resting tremor, rigidity, and bradykinetic movements. Symptomatic treatments, such as levodopa therapies, may permit the patient better mobility. Mechanism of Action Current evidence indicates that symptoms of Parkinson’s disease are related to depletion of dopamine in the corpus striatum.

Pharmacokinetic profiling of Carbidopa demonstrates an elimination half-life of Biphasic clearance with 8–15 hr terminal t½ under Carbidopa maintenance with Systemic oral bioavailability of roughly 50%–62% following first-pass transit systemic bioavailability; clearance proceeds via hepatic enzymatic processing and renal clearance proportional to creatinine clearance (carbidopa), reaching clinical onset within initial pharmacodynamic onset within 1–3 hrs (peak cmax) for carbidopa.

Titration schedules for Carbidopa (25 mg/1 Tablet → 25mg Tablet) require excluding contraindications carbidopa tablets are contraindicated in patients with known hypersensitivity to any component of this drug.; nonselective monoamine oxidase (mao) inhibitors are contraindicated for use with levodopa or carbidopa-levodopa combination products with or without carbidopa.; real-world patient logs highlight nausea (10.4% of reports), headache (7.1% of reports), dizziness (6.2% of reports) alongside a 79.00% Worth-It consensus.

Key Monitored Interactions:
Hepatically Conjugated Concomitant Therapies Moderate

Assess baseline hepatic panels and interacting prescriptions when starting Carbidopa (aromatic amino acid decarboxylation inhibitor).

Gastric pH-Altering Agents (PPIs & H2 Blockers) Minor

Shifts in intragastric pH can modestly influence dissolution kinetics of oral Carbidopa dosage forms.
