Carvedilol Phosphate Mechanism, CYP450 Pathways & DailyMed Monograph
Carvedilol phosphate extended-release capsules are an alpha-/beta-adrenergic blocking agent indicated for the treatment of: mild to severe chronic heart failure. ( 1.1 ) left ventricular dysfunction following myocardial infarction in clinically stable patients. ( 1.2 ) hypertension. ( 1.3 ) 1.1 Heart Failure Carvedilol phosphate extended-release capsules are indicated for the treatment of mild-to-severe chronic heart failure of ischemic or cardiomyopathic origin, usually in addition to diuretics, ACE inhibitors, and digitalis, to increase survival and, also, to reduce the risk of hospitalization .
12.1 Mechanism of Action Carvedilol is a racemic mixture in which nonselective beta-adrenoreceptor blocking activity is present in the S(–) enantiomer and alpha 1 -adrenergic blocking activity is present in both R(+) and S(–) enantiomers at equal potency. Carvedilol has no intrinsic sympathomimetic activity.
Following administration (40 mg/1 Capsule, Extended Release), Carvedilol Phosphate achieves ~25% systemic bioavailability (Carvedilol Phosphate) bioavailability and maximum serum exposure occurs 2–5 hours after carvedilol phosphate dosing onset, governed by a Steady-state plasma equilibrium reached over 9–20 hour t½ intervals for Carvedilol Phosphate terminal half-life through predominantly hepatic biotransformation with renal elimination of polar carvedilol phosphate conjugates.
Before initiating Carvedilol Phosphate at 40 mg/1 Capsule, Extended Release, clinical safety checklists rule out carvedilol phosphate extended-release capsules are contraindicated in the following conditions: bronchial asthma or related bronchospastic conditions.; deaths from status asthmaticus have been reported following single doses of immediate-release carvedilol.; second- or third-degree av block.. Logged tolerability patterns center on nausea (8.8% of reports), dizziness (8.4% of reports), fatigue (8.2% of reports).
Chronic enzyme induction shortens Carvedilol Phosphate persistence in plasma and reduces steady-state exposure.
Transporter-mediated clearance of P-gp substrates may slow during concurrent Carvedilol Phosphate administration.
