Denosumab Mechanism, CYP450 Pathways & DailyMed Monograph
Bilprevda is a RANK ligand (RANKL) inhibitor indicated for: Prevention of skeletal-related events in patients with multiple myeloma and in patients with bone metastases from solid tumors. ( 1.1 ) Treatment of adults and skeletally mature adolescents with giant cell tumor of bone that is unresectable or where surgical resection is likely to result in severe morbidity. ( 1.2 , 14.3 ) Treatment of hypercalcemia of malignancy refractory to bisphosphonate therapy.
12.1 Mechanism of Action Denosumab products bind to RANKL, a transmembrane or soluble protein essential for the formation, function, and survival of osteoclasts, the cells responsible for bone resorption, thereby modulating calcium release from bone. Increased osteoclast activity, stimulated by RANKL, is a mediator of bone pathology in solid tumors with osseous metastases.Similarly, giant cell tumors of bone consist of stromal cells expressing RANKL and osteoclast-like giant cells expressing RANK receptor, and signaling through the RANK receptor contributes to osteolysis and tumor growth.
Systemic exposure to Denosumab reflects ~62% systemic bioavailability (Denosumab) absorption kinetics and dual hepatorenal clearance pathway combining glucuronidation and urinary filtration of denosumab, sustaining receptor engagement across a Mean elimination half-life of 7 to 18 hours across adult Denosumab dosing half-life window (rapid intravascular/parenteral onset within 15–45 minutes after denosumab administration response profile).
Dosing protocols (60 mg/mL Injection, Solution or 120 mg/1.7mL Injection, Solution or 70 mg/mL Injection or 60 mg/mL Injection or 120 mg/1.7mL Injection) balance efficacy (81.00% patient satisfaction across 17 logs) against nausea (10.3% of reports), fatigue (8% of reports), headache (7% of reports), with strict avoidance in ( 4.1 ) known clinically significant hypersensitivity to denosumab products.; ( 4.2 ) 4.1 hypocalcemia pre-existing hypocalcemia must be corrected prior to initiating therapy with bilprevda .; 4.2 hypersensitivity bilprevda is contraindicated in patients with known clinically significant hypersensitivity to denosumab products ..
Separate Denosumab ingestion from mineral supplements or antacids to preserve gastrointestinal bioavailability.
Reconcile eGFR-adjusted medications before escalating Denosumab in polypharmacy regimens.
