Elagolix and Estradiol and Norethisterone Mechanism, CYP450 Pathways & DailyMed Monograph

RxCUI 2371764 • NDC 0074-1017

1 Indications and usage oriahnn: is indicated for the management of heavy menstrual bleeding associated with uterine leiomyomas (fibroids) in premenopausal women. Limitation of Use: Use of ORIAHNN should be limited to 24 months due to the risk of continued bone loss, which may not be reversible . ORIAHNN is a combination of elagolix, a gonadotropin-releasing hormone (GnRH) receptor antagonist, estradiol, an estrogen, and norethindrone acetate, a progestin, indicated for the management of heavy menstrual bleeding associated with uterine leiomyomas (fibroids) in premenopausal women.

12.1 Mechanism of Action ORIAHNN combines elagolix and estradiol/norethindrone acetate (E2/NETA), a combination of estrogen and progestin. Elagolix is a GnRH receptor antagonist that inhibits endogenous GnRH signaling by binding competitively to GnRH receptors in the pituitary gland. Administration of elagolix results in dose-dependent suppression of luteinizing hormone (LH) and follicle-stimulating hormone (FSH), leading to decreased blood concentrations of the ovarian sex hormones estradiol and progesterone and reduces bleeding associated with uterine fibroids.

Systemic exposure to Elagolix and Estradiol and Norethisterone reflects Systemic oral bioavailability of roughly 63%–75% following first-pass transit absorption kinetics and primary cyp2c19 & p-gp cytochrome processing paired with glucuronide elimination for elagolix and estradiol and norethisterone, sustaining receptor engagement across a Biphasic clearance with 5–11 hr terminal t½ under Elagolix and Estradiol and Norethisterone maintenance half-life window (initial pharmacodynamic onset within 1–3 hrs (peak cmax) for elagolix and estradiol and norethisterone response profile).

Dosing protocols (Kit or 300 mg or 1 mg or 0.5 mg) balance efficacy (85.00% patient satisfaction across 20 logs) against headache (11.4% of reports), nausea (7.4% of reports), fatigue (6.9% of reports), with strict avoidance in 4 contraindications oriahnn is contraindicated in women: with a high risk of arterial, venous thrombotic, or thromboembolic disorders .; severe drug-induced hypersensitivity to elagolix and estradiol and norethisterone or inactive tablet/vehicle binders.
