Eribulin Mesylate Mechanism, CYP450 Pathways & DailyMed Monograph

RxCUI 1045456 • NDC 68001-656

Eribulin Mesylate Injection is a microtubule inhibitor indicated for the treatment of patients with: Metastatic breast cancer who have previously received at least two chemotherapeutic regimens for the treatment of metastatic disease. Prior therapy should have included an anthracycline and a taxane in either the adjuvant or metastatic setting. ( 1.1 ) Unresectable or metastatic liposarcoma who have received a prior anthracycline-containing regimen.

12.1 Mechanism of Action Eribulin inhibits the growth phase of microtubules without affecting the shortening phase and sequesters tubulin into nonproductive aggregates. Eribulin exerts its effects via a tubulin-based antimitotic mechanism leading to G 2 /M cell-cycle block, disruption of mitotic spindles, and, ultimately, apoptotic cell death after prolonged mitotic blockage. In addition, eribulin treatment of human breast cancer cells caused changes in morphology and gene expression as well as decreased migration and invasiveness in vitro .

Systemic exposure to Eribulin Mesylate reflects Complete (~100%) parenteral systemic delivery for Eribulin Mesylate (.5 mg/mL Injection) absorption kinetics and phase i cyp3a4, p-gp hepatic clearance and subsequent renal metabolite filtration, sustaining receptor engagement across a 40 hours terminal elimination (Eribulin Mesylate SPL) half-life window (rapid intravascular/parenteral onset within 15–45 minutes after eribulin mesylate administration response profile).

Dosing protocols (.5 mg/mL Injection or .5 mg/mL Solution or 1 mg/2mL Injection) balance efficacy (89.00% patient satisfaction across 15 logs) against nausea (11.1% of reports), fatigue (8.8% of reports), headache (8.4% of reports), with strict avoidance in contraindicated in patients with prior serious allergic response to eribulin mesylate (ndc 68001-656); severe baseline organ impairment where eribulin mesylate accumulation exceeds safe therapeutic margins.

CYP450 & Metabolic Pathways: CYP3A4 P-gp
Key Monitored Interactions:
Potent CYP3A4 Inhibitors (Azoles, Clarithromycin, Ritonavir) Major

Azole antifungals or ritonavir can markedly raise Eribulin Mesylate plasma peaks; dose reduction may be warranted.

Hepatic CYP Inducers (Rifampin, Carbamazepine, Phenytoin) Moderate

Rifampin or anticonvulsant inducers can diminish Eribulin Mesylate systemic exposure and clinical response.

Digoxin & P-Glycoprotein Efflux Substrates Moderate

Monitor serum digoxin concentrations when initiating or adjusting Eribulin Mesylate due to transporter overlap.

Half-Life
40 hours terminal elimination (Eribulin Mesylate SPL)
