Exemestane Mechanism, CYP450 Pathways & DailyMed Monograph

RxCUI 310261 • NDC 63629-2056

Exemestane tablets are an aromatase inhibitor indicated for: adjuvant treatment of postmenopausal women with estrogen-receptor positive early breast cancer who have received two to three years of tamoxifen and are switched to exemestane tablets for completion of a total of five consecutive years of adjuvant hormonal therapy ( 14.1 ). treatment of advanced breast cancer in postmenopausal women whose disease has progressed following tamoxifen therapy ( 14.2 ).

12.1 Mechanism of Action Breast cancer cell growth may be estrogen-dependent. Aromatase is the principal enzyme that converts androgens to estrogens both in pre- and postmenopausal women. While the main source of estrogen (primarily estradiol) is the ovary in premenopausal women, the principal source of circulating estrogens in postmenopausal women is from conversion of adrenal and ovarian androgens (androstenedione and testosterone) to estrogens (estrone and estradiol) by the aromatase enzyme in peripheral tissues.

Systemic exposure to Exemestane reflects Oral fraction absorbed ranges from 76% to 88% (Exemestane 25) absorption kinetics and dual hepatorenal clearance pathway combining glucuronidation and urinary filtration of exemestane, sustaining receptor engagement across a 24 hours terminal elimination (Exemestane SPL) half-life window (oral tmax achieved within 2 to 3 hours following exemestane ingestion response profile).

Dosing protocols (25 mg/1 Tablet, Film Coated or 25 mg/1 Tablet or 25 mg/1 Tablet, Sugar Coated or 25mg Tablet) balance efficacy (84.00% patient satisfaction across 16 logs) against fatigue (16% of reports), headache (13% of reports), insomnia (12% of reports), with strict avoidance in exemestane tablets are contraindicated in patients with a known hypersensitivity to the drug or to any of the excipients.; patients with a known hypersensitivity to the drug or to any of the excipients ( 4 )..

Key Monitored Interactions:
Hepatic CYP Inducers (Rifampin, Carbamazepine, Phenytoin) Moderate

Rifampin or anticonvulsant inducers can diminish Exemestane systemic exposure and clinical response.

Renally Cleared Narrow-Index Co-Medications Moderate

Reconcile eGFR-adjusted medications before escalating Exemestane in polypharmacy regimens.
