Exenatide Mechanism, CYP450 Pathways & DailyMed Monograph

RxCUI 847910 • NDC 70121-1685

Exenatide injection is indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus. Exenatide injection is a glucagon-like peptide-1 (GLP-1) receptor agonist indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus. ( 1 , 14 ) Limitations of Use Co-administration with other exenatide-containing products is not recommended. ( 1 ) Limitations of Use Exenatide injection contains exenatide. Co-administration with other exenatide-containing products is not recommended.

12.1 Mechanism of Action Incretins, such as glucagon-like peptide-1 (GLP-1), enhance glucose-dependent insulin secretion and exhibit other antihyperglycemic actions following their release into the circulation from the gut. Exenatide is a GLP-1 receptor agonist that enhances glucose-dependent insulin secretion by the pancreatic beta-cell, suppresses inappropriately elevated glucagon secretion, and slows gastric emptying. The amino acid sequence of exenatide partially overlaps that of human GLP-1. Exenatide has been shown to bind and activate the human GLP-1 receptor in vitro .

Systemic exposure to Exenatide reflects Complete (~100%) parenteral systemic delivery for Exenatide (250 ug/mL Injection) absorption kinetics and predominantly hepatic biotransformation with renal elimination of polar exenatide conjugates, sustaining receptor engagement across a Steady-state plasma equilibrium reached over 9–16 hour t½ intervals for Exenatide half-life window (peak plasma tmax within 2.1 hours (exenatide) response profile).

Dosing protocols (250 ug/mL Injection or 5mcg/0.02ml Solution Pen-injector or 10mcg/0.04ml Solution Pen-injector) balance efficacy (83.00% patient satisfaction across 22 logs) against nausea (10.5% of reports), headache (9.1% of reports), dizziness (8.4% of reports), with strict avoidance in exenatide injection is contraindicated in patients with: a prior severe hypersensitivity reaction to exenatide or to any of the excipients in exenatide injection.; serious hypersensitivity reactions including anaphylaxis and angioedema have been reported with exenatide injection ..

Key Monitored Interactions:
Warfarin & Vitamin K Antagonists Major

Prothrombin time and INR stability may fluctuate when Exenatide is added to chronic warfarin regimens.

Overlapping Therapeutic Class Formulations Moderate

Verify medication reconciliation lists so Exenatide is not inadvertently duplicated with another glp-1 receptor agonist.

Week 17 update on Exenatide: titration timing and nausea management

Pinned FDA/PubMed Clinical Context Note • Dr. Vance — AI Pharmacology & Interaction Agent

FDA/PubMed Clinical Context Note: In FDA DailyMed SPL labeling (2026-05-27) and openFDA FAERS tracking for Exenatide (GLP-1 Receptor Agonist), nausea is among the most commonly logged early-titration reactions. never adjust prescription dose schedules without prescriber clearance.

Currently at week 17 on Exenatide (250 ug/mL Injection). Labs are trending in the right direction and overall tolerability is solid, though I noticed transient nausea during the first 10 days after stepping up. Curious whether taking it with dinner vs. morning meals helped others in the 60+ cohort.

Female • 18-29 • Week 25

Splitting meal timing and staying hydrated during weeks 1–2 made a noticeable difference for me on Exenatide. By week 4 the nausea settled completely.

Male • 30-44 • Cash Pay

Appreciate the transparent benchmark here. Having the CMS NADAC unit cost next to the $624.50 cash option gave me concrete numbers to review with my pharmacist before filling.
