Fruquintinib Mechanism, CYP450 Pathways & DailyMed Monograph

RxCUI 2670184 • NDC 63020-210

1. Indications and usage fruzaqla: is indicated for the treatment of adult patients with metastatic colorectal cancer (mCRC) who have been previously treated with fluoropyrimidine‑, oxaliplatin‑, and irinotecan‑based chemotherapy, an anti‑VEGF therapy, and, if RAS wild‑type and medically appropriate, an anti-EGFR therapy.

12.1. Mechanism of Action Fruquintinib is a small molecule kinase inhibitor of vascular endothelial growth factor receptors (VEGFR)-1, -2, and -3 with IC 50 values of 33, 35, and 0.5 nM, respectively. In vitro studies showed fruquintinib inhibited VEGF-mediated endothelial cell proliferation and tubular formation. In vitro and in vivo studies showed fruquintinib inhibited VEGF-induced VEGFR-2 phosphorylation. In vivo studies showed fruquintinib inhibited tumor growth in a tumor xenograft mouse model of colon cancer.

Pharmacokinetic profiling of Fruquintinib demonstrates an elimination half-life of Terminal t½ estimated at 7–19 hrs depending on renal/hepatic reserve (Fruquintinib) with F ≈ 62%–74% systemic availability after standard oral Fruquintinib administration systemic bioavailability; clearance proceeds via mixed metabolic conjugation and unchanged urinary excretion across fruquintinib regimens, reaching clinical onset within time to peak concentration spans 1 to 3 hrs under fruquintinib maintenance.

Titration schedules for Fruquintinib (1 mg/1 Capsule → 5 mg/1 Capsule) require excluding contraindications none.; severe drug-induced hypersensitivity to fruquintinib or inactive tablet/vehicle binders; real-world patient logs highlight nausea (9.2% of reports), diarrhea (8.5% of reports), headache (7.4% of reports) alongside a 84.00% Worth-It consensus.

Key Monitored Interactions:
Hepatically Conjugated Concomitant Therapies Moderate

Assess baseline hepatic panels and interacting prescriptions when starting Fruquintinib (fda prescription therapeutic agent).

Herbal CYP/Transporter Supplements (St. John’s Wort, Grapefruit) Minor

Botanical enzyme modulators may alter oral first-pass exposure of Fruquintinib; disclose all supplements.
