Glimepiride Mechanism, CYP450 Pathways & DailyMed Monograph

RxCUI 199246 • NDC 50090-6426

Glimepiride tablets are indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus . Limitations of Use Glimepiride tablets should not be used for the treatment of type 1 diabetes mellitus or diabetic ketoacidosis, as it would not be effective in these settings. Glimepiride is a sulfonylurea indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus (1) . Limitations of Use: Not for treating type 1 diabetes mellitus or diabetic ketoacidosis ( 1 ).

12.1 Mechanism of Action Glimepiride primarily lowers blood glucose by stimulating the release of insulin from pancreatic beta cells. Sulfonylureas bind to the sulfonylurea receptor in the pancreatic beta-cell plasma membrane, leading to closure of the ATP-sensitive potassium channel, thereby stimulating the release of insulin.

With Terminal t½ estimated at 10–19 hrs depending on renal/hepatic reserve (Glimepiride) residence kinetics and F ≈ 47%–59% systemic availability after standard oral Glimepiride administration bioavailability, Glimepiride undergoes mixed metabolic conjugation and unchanged urinary excretion across glimepiride regimens and establishes therapeutic activity over time to peak concentration spans 1 to 2 hrs under glimepiride maintenance.

Standard adult regimens span 1 mg/1 Tablet, 4 mg/1 Tablet, 2 mg/1 Tablet, 6 mg/1 Tablet, 3 mg/1 Tablet. Prescribers screen specifically against glimepiride tablets are contraindicated in patients with a history of a hypersensitivity reaction to: glimepiride or any of the product’s ingredients .; sulfonamide derivatives: patients who have developed an allergic reaction to sulfonamide derivatives may develop an allergic reaction to glimepiride tablets., while community micro-logs most frequently note nausea (10.6% of reports), headache (9.2% of reports), dizziness (8.5% of reports).

Key Monitored Interactions:
RAAS Modulators & Potassium-Sparing Diuretics Moderate

Combining Glimepiride with ACE inhibitors, ARBs, or aldosterone blockers can shift serum potassium and eGFR.

Duplicate Sulfonylurea Agents Moderate

Avoid overlapping sulfonylurea prescriptions alongside Glimepiride to prevent cumulative receptor toxicity.
