Hydralazine Hydrochloride and Isosorbide Dinitrate Mechanism, CYP450 Pathways & DailyMed Monograph

RxCUI 905377 • NDC 72162-2343

Isosorbide dinitrate and hydralazine hydrochloride tablets are a combination of isosorbide dinitrate, a nitrate vasodilator, and hydralazine hydrochloride, an arteriolar vasodilator, indicated for: the treatment of heart failure as an adjunct therapy to standard therapy in self-identified black patients to improve survival, prolong time to hospitalization for heart failure and to improve patient-reported functional status ( 1.1 ) Limitations of use: There is little experience in patients with NYHA class IV heart failure ( 1.2 ) 1.1 Treatment of Heart Failure in Self-identified Black Patients Isosorbide dinitrate.

12.1 Mechanism of Action The mechanism of action underlying the beneficial effects of isosorbide dinitrate and hydralazine hydrochloride tablets in the treatment of heart failure has not been established. Isosorbide dinitrate is a vasodilator affecting both arteries and veins. Its dilator properties result from the release of nitric oxide and the subsequent activation of guanylyl cyclase, and ultimate relaxation of vascular smooth muscle. Several well-controlled clinical trials have used exercise testing to assess the anti-anginal efficacy of chronically-delivered nitrates.

Following administration (37.5 mg/1 / 20 mg/1 Tablet, Film Coated), Hydralazine Hydrochloride and Isosorbide Dinitrate achieves Estimated 49%–61% oral systemic absorption for Hydralazine Hydrochloride and Isosorbide Dinitrate bioavailability and peak plasma concentration (tmax) reached 1–4 hours post-dose for hydralazine hydrochloride and isosorbide dinitrate onset, governed by a Approximately 3–12 hours terminal plasma half-life for Hydralazine Hydrochloride and Isosorbide Dinitrate terminal half-life through phase i/ii hepatic conjugation of hydralazine hydrochloride and isosorbide dinitrate coupled with glomerular and tubular renal excretion.

Before initiating Hydralazine Hydrochloride and Isosorbide Dinitrate at 37.5 mg/1 / 20 mg/1 Tablet, Film Coated, clinical safety checklists rule out isosorbide dinitrate and hydralazine hydrochloride tablets are contraindicated in patients who are allergic to organic nitrates.; do not use isosorbide dinitrate and hydralazine hydrochloride tablets in patients who are taking pde-5 inhibitors, such as avanafil, sildenafil, tadalafil, or vardenafil.. Logged tolerability patterns center on headache (11.2% of reports), nausea (7.5% of reports), dizziness (6.9% of reports).

Key Monitored Interactions:
Duplicate Nitrate Vasodilator Agents Moderate

Avoid overlapping nitrate vasodilator prescriptions alongside Hydralazine Hydrochloride and Isosorbide Dinitrate to prevent cumulative receptor toxicity.

Gastric pH-Altering Agents (PPIs & H2 Blockers) Minor

Shifts in intragastric pH can modestly influence dissolution kinetics of oral Hydralazine Hydrochloride and Isosorbide Dinitrate dosage forms.
