Hydroxychloroquine Sulfate Mechanism, CYP450 Pathways & DailyMed Monograph

RxCUI 979092 • NDC 50268-412

Indications and usage: Malaria Hydroxychloroquine Sulfate Tablets are indicated for the treatment of uncomplicated malaria due to P. falciparum, P. malariae, P. ovale , and P. vivax. Hydroxychloroquine Sulfate Tablets are indicated for the prophylaxis of malaria in geographic areas where chloroquine resistance is not reported. Limitations of Use in Malaria Hydroxychloroquine Sulfate Tablets are not recommended for the treatment of complicated malaria. Hydroxychloroquine Sulfate Tablets are not effective against chloroquine or hydroxychloroquine- resistant strains of Plasmodium species .

CLINICAL PHARMACOLOGY: Pharmacokinetics: Following a single 200 mg oral dose of hydroxychloroquine sulfate tablets to healthy male volunteers, the mean peak blood concentration of hydroxychloroquine was 129.6 ng/mL, reached in 3.26 hours with a half-life of 537 hours (22.4 days). In the same study, the plasma peak concentration was 50.3 ng/mL reached in 3.74 hours with a half-life of 2963 hours (123.5 days). Urine hydroxychloroquine levels were still detectable after 3 months with approximately 10% of the dose excreted as the parent drug. Results following a single dose of a 200 mg tablet versus i.v.

Following administration (200 mg/1 Tablet, Film Coated), Hydroxychloroquine Sulfate achieves Systemic oral bioavailability of roughly 73%–85% following first-pass transit bioavailability and initial pharmacodynamic onset within 1–3 hrs (peak cmax) for hydroxychloroquine sulfate onset, governed by a 537 hours terminal elimination (Hydroxychloroquine Sulfate SPL) terminal half-life through hepatic enzymatic processing and renal clearance proportional to creatinine clearance (hydroxychloroquine sulfate).

Before initiating Hydroxychloroquine Sulfate at 200 mg/1 Tablet, Film Coated, clinical safety checklists rule out contraindications use of hydroxychloroquine sulfate tablets is contraindicated in patients with known hypersensitivity to 4-aminoquinoline compounds.; prior anaphylaxis, angioedema, or severe hypersensitivity to hydroxychloroquine sulfate or hydroxychloroquine sulfate excipients. Logged tolerability patterns center on nausea (8.7% of reports), headache (8.3% of reports), fatigue (5.7% of reports).

Key Monitored Interactions:
Digoxin & P-Glycoprotein Efflux Substrates Moderate

Hydroxychloroquine Sulfate may compete for P-gp efflux transport, raising narrow-therapeutic-index digoxin serum levels.

Duplicate Antimalarial Agents Moderate

Avoid overlapping antimalarial prescriptions alongside Hydroxychloroquine Sulfate to prevent cumulative receptor toxicity.
