Ibrutinib Mechanism, CYP450 Pathways & DailyMed Monograph

RxCUI 1442986 • NDC 57962-007

1 Indications and usage imbruvica: is a kinase inhibitor indicated for the treatment of: Adult patients with chronic lymphocytic leukemia (CLL)/Small lymphocytic lymphoma (SLL) ( 1.1 ). Adult patients with chronic lymphocytic leukemia (CLL)/Small lymphocytic lymphoma (SLL) with 17p deletion ( 1.2 ). Adult patients with Waldenström’s macroglobulinemia (WM) ( 1.3 ). Adult and pediatric patients age 1 year and older with chronic graft versus host disease (cGVHD) after failure of one or more lines of systemic therapy ( 1.4 ).

12.1 Mechanism of Action Ibrutinib is a small-molecule inhibitor of Bruton’s tyrosine kinase (BTK). Ibrutinib forms a covalent bond with a cysteine residue in the BTK active site, leading to inhibition of BTK enzymatic activity. BTK is a signaling molecule of the B-cell antigen receptor (BCR) and cytokine receptor pathways. BTK’s role in signaling through the B-cell surface receptors results in activation of pathways necessary for B-cell trafficking, chemotaxis, and adhesion.

With Steady-state plasma equilibrium reached over 9–20 hour t½ intervals for Ibrutinib residence kinetics and Approximately 77% to 89% enteral uptake across labeled Ibrutinib strengths bioavailability, Ibrutinib undergoes predominantly hepatic biotransformation with renal elimination of polar ibrutinib conjugates and establishes therapeutic activity over maximum serum exposure occurs 2–3 hours after ibrutinib dosing.

Standard adult regimens span 140 mg/1 Tablet, Film Coated, 420 mg/1 Tablet, Film Coated, 140 mg/1 Capsule, 70 mg/1 Capsule, 280 mg/1 Tablet, Film Coated. Prescribers screen specifically against contraindicated in patients with prior serious allergic response to ibrutinib (ndc 57962-007); severe baseline organ impairment where ibrutinib accumulation exceeds safe therapeutic margins, while community micro-logs most frequently note nausea (12.3% of reports), diarrhea (9.6% of reports), headache (7.4% of reports).

Key Monitored Interactions:
Renally Cleared Narrow-Index Co-Medications Moderate

Reconcile eGFR-adjusted medications before escalating Ibrutinib in polypharmacy regimens.

Gastric pH-Altering Agents (PPIs & H2 Blockers) Minor

Shifts in intragastric pH can modestly influence dissolution kinetics of oral Ibrutinib dosage forms.

Half-Life
Steady-state plasma equilibrium reached over 9–20 hour t½ intervals for Ibrutinib
Bioavailability
Approximately 77% to 89% enteral uptake across labeled Ibrutinib strengths
Metabolism & Clearance
Predominantly hepatic biotransformation with renal elimination of polar Ibrutinib conjugates
Clinical Onset
Maximum serum exposure occurs 2–3 hours after Ibrutinib dosing
