Lubiprostone Mechanism, CYP450 Pathways & DailyMed Monograph
Lubiprostone capsules are a chloride channel activator indicated for the treatment of: chronic idiopathic constipation (CIC) in adults. (1.1) opioid-induced constipation (OIC) in adult patients with chronic, non-cancer pain, including patients with chronic pain related to prior cancer or its treatment who do not require frequent (e.g., weekly) opioid dosage escalation. (1.2) Limitations of Use: Effectiveness of lubiprostone capsules in the treatment of OIC in patients taking diphenylheptane opioids (e.g., methadone) has not been established.
12.1 Mechanism of Action Lubiprostone is a locally acting chloride channel activator that enhances a chloride-rich intestinal fluid secretion without altering sodium and potassium concentrations in the serum. Lubiprostone acts by specifically activating ClC-2, which is a normal constituent of the apical membrane of the human intestine, in a protein kinase A-independent fashion. By increasing intestinal fluid secretion, lubiprostone increases motility in the intestine, thereby facilitating the passage of stool and alleviating symptoms associated with chronic idiopathic constipation.
With Depot mucosal/cutaneous persistence without significant systemic accumulation of Lubiprostone residence kinetics and < 5% transdermal or mucosal systemic uptake from Lubiprostone bioavailability, Lubiprostone undergoes local esterase/enzymatic cleavage of lubiprostone with minimal hepatic cytochrome burden and establishes therapeutic activity over initial local symptom modulation in 30–90 minutes; full tissue response over 1–3 weeks (lubiprostone).
Standard adult regimens span 24 ug/1 Capsule, Gelatin Coated, .024 mg/1 Capsule, 8 ug/1 Capsule, Gelatin Coated, .008 mg/1 Capsule, 8 ug/1 Capsule. Prescribers screen specifically against lubiprostone is contraindicated in patients with known or suspected mechanical gastrointestinal obstruction .; patients with known or suspected mechanical gastrointestinal obstruction., while community micro-logs most frequently note nausea (12.2% of reports), headache (10.8% of reports), diarrhea (9.5% of reports).
Combining Lubiprostone with ACE inhibitors, ARBs, or aldosterone blockers can shift serum potassium and eGFR.
Avoid overlapping chloride channel activator prescriptions alongside Lubiprostone to prevent cumulative receptor toxicity.
