Melphalan Hydrochloride Mechanism, CYP450 Pathways & DailyMed Monograph

RxCUI 311487 • NDC 43598-392

Indications and usage: Melphalan hydrochloride for injection is indicated for the palliative treatment of patients with multiple myeloma for whom oral therapy is not appropriate.

CLINICAL PHARMACOLOGY Melphalan is an alkylating agent of the bischloroethylamine type. As a result, its cytotoxicity appears to be related to the extent of its interstrand cross-linking with DNA, probably by binding at the N 7 position of guanine. Like other bifunctional alkylating agents, it is active against both resting and rapidly dividing tumor cells. Pharmacokinetics Following injection, drug plasma concentrations declined rapidly in a biexponential manner with distribution phase and terminal elimination phase half-lives of approximately 10 and 75 minutes, respectively.

Pharmacokinetic profiling of Melphalan Hydrochloride demonstrates an elimination half-life of Terminal t½ estimated at 4–10 hrs depending on renal/hepatic reserve (Melphalan Hydrochloride) with F ≈ 47%–59% systemic availability after standard oral Melphalan Hydrochloride administration systemic bioavailability; clearance proceeds via mixed metabolic conjugation and unchanged urinary excretion across melphalan hydrochloride regimens, reaching clinical onset within time to peak concentration spans 1 to 2 hrs under melphalan hydrochloride maintenance.

Titration schedules for Melphalan Hydrochloride (Kit → 16 mg → 50% → 30 mg) require excluding contraindications melphalan hydrochloride for injection should not be used in patients whose disease has demonstrated prior resistance to this agent.; patients who have demonstrated hypersensitivity to melphalan hydrochloride for injection should not be given the drug.; real-world patient logs highlight nausea (9.4% of reports), headache (7.5% of reports), dizziness (5.3% of reports) alongside a 90.00% Worth-It consensus.
