Methylphenidate Hydrochloride Extended Release Mechanism, CYP450 Pathways & DailyMed Monograph

RxCUI 1091145 • NDC 10702-075

Methylphenidate hydrochloride extended-release tablets are indicated for the treatment of: ; Attention Deficit Hyperactivity Disorders (ADHD) in pediatric patients 6 years and older and adults ; Narcolepsy Limitations of Use: The use of Methylphenidate hydrochloride is not recommended in pediatric patients younger than 6 years of age because they had higher plasma exposure and a higher incidence of adverse reactions (e.g., weight loss) than patients 6 years and older at the same dosage .

12.1 Mechanism of Action Methylphenidate hydrochloride is a CNS stimulant. The mode of therapeutic action in ADHD and narcolepsy is not known. 12.2 Pharmacodynamics Methylphenidate is a racemic mixture comprised of the d- and l-threo enantiomers. The d-threo enantiomer is more pharmacologically active than the l-threo enantiomer. Methylphenidate blocks the reuptake of norepinephrine and dopamine into the presynaptic neuron and increase the release of these monoamines into the extraneuronal space.

Following administration (10 mg/1 Tablet), Methylphenidate Hydrochloride Extended Release achieves Approximately 75% to 87% enteral uptake across labeled Methylphenidate Hydrochloride Extended Release strengths bioavailability and maximum serum exposure occurs 2–4 hours after methylphenidate hydrochloride extended release dosing onset, governed by a Steady-state plasma equilibrium reached over 3–12 hour t½ intervals for Methylphenidate Hydrochloride Extended Release terminal half-life through predominantly hepatic biotransformation with renal elimination of polar methylphenidate hydrochloride extended release conjugates.

Before initiating Methylphenidate Hydrochloride Extended Release at 10 mg/1 Tablet, clinical safety checklists rule out 4 contraindications; hypersensitivity to methylphenidate or other components of methylphenidate hydrochloride extended-release tablets.; hypersensitivity reactions, such as angioedema and anaphylactic reactions, have been reported in patients treated with methylphenidate .. Logged tolerability patterns center on nausea (7.6% of reports), headache (6.6% of reports), dizziness (5.9% of reports).
