Mexiletine Hydrochloride Mechanism, CYP450 Pathways & DailyMed Monograph

RxCUI 1362706 • NDC 63629-9179

Indications and usage: Mexiletine hydrochloride capsules, USP are indicated for the treatment of documented ventricular arrhythmias, such as sustained ventricular tachycardia, that, in the judgment of the physician, are life-threatening. Because of the proarrhythmic effects of mexiletine, its use with lesser arrhythmias is generally not recommended. Treatment of patients with asymptomatic ventricular premature contractions should be avoided. Initiation of mexiletine treatment, as with other antiarrhythmic agents used to treat life-threatening arrhythmias, should be carried out in the hospital.

CLINICAL PHARMACOLOGY Mechanism of Action Mexiletine hydrochloride is a local anesthetic, antiarrhythmic agent, structurally similar to lidocaine, but orally active. In animal studies, mexiletine has been shown to be effective in the suppression of induced ventricular arrhythmias, including those induced by glycoside toxicity and coronary artery ligation. Mexiletine, like lidocaine, inhibits the inward sodium current, thus reducing the rate of rise of the action potential, Phase 0. Mexiletine decreased the effective refractory period (ERP) in Purkinje fibers.

Pharmacokinetic profiling of Mexiletine Hydrochloride demonstrates an elimination half-life of Elimination kinetics span 8 to 18 hours per Mexiletine Hydrochloride clinical pharmacology with Consistent 78%–90% gastrointestinal absorption profile for Mexiletine Hydrochloride systemic bioavailability; clearance proceeds via systemic clearance of mexiletine hydrochloride integrates hepatic phase ii conjugation with renal filtration, reaching clinical onset within cmax attained 2–5 hours post-administration of mexiletine hydrochloride.

Titration schedules for Mexiletine Hydrochloride (250 mg/1 Capsule → 200 mg/1 Capsule → 150 mg/1 Capsule → 150mg Capsule → 200mg Capsule) require excluding previous anaphylactoid or exfoliative reaction attributed to mexiletine hydrochloride; clinical scenarios where antiarrhythmic exposure from mexiletine hydrochloride poses unacceptable risk; real-world patient logs highlight nausea (8.8% of reports), headache (7.2% of reports), dizziness (6.2% of reports) alongside a 79.00% Worth-It consensus.

Key Monitored Interactions:
QTc-Prolonging Antiarrhythmics (Amiodarone, Sotalol) Major

Electrophysiologic additive QTc lengthening warrants caution when Mexiletine Hydrochloride is combined with amiodarone or sotalol.

Overlapping Therapeutic Class Formulations Moderate

Verify medication reconciliation lists so Mexiletine Hydrochloride is not inadvertently duplicated with another antiarrhythmic.
