Naloxone Hydrochloride Mechanism, CYP450 Pathways & DailyMed Monograph

RxCUI 1191222 • NDC 51662-1673

Naloxone Hydrochloride Injection is indicated for the complete or partial reversal of opioid depression, including respiratory depression, induced by natural and synthetic opioids including propoxyphene, methadone, and certain mixed agonist-antagonist analgesics: nalbuphine, pentazocine, butorphanol, and cyclazocine. Naloxone hydrochloride is also indicated for the diagnosis of suspected or known acute opioid overdosage. Naloxone may be useful as an adjunctive agent to increase blood pressure in the management of septic shock .

Complete or Partial Reversal of Opioid Depression Naloxone prevents or reverses the effects of opioids including respiratory depression, sedation and hypotension. Also, Naloxone can reverse the psychotomimetic and dysphoric effects of agonist-antagonists such as pentazocine. Naloxone is an essentially pure opioid antagonist, i.e., it does not possess the "agonistic" or morphine-like properties characteristic of other opioid antagonists. When administered in usual doses and in the absence of opioids or agonistic effects of other opioid antagonists, it exhibits essentially no pharmacologic activity.

Systemic exposure to Naloxone Hydrochloride reflects Complete (~100%) parenteral systemic delivery for Naloxone Hydrochloride (1 mg/mL Injection) absorption kinetics and phase i/ii hepatic conjugation of naloxone hydrochloride coupled with glomerular and tubular renal excretion, sustaining receptor engagement across a Approximately 6–17 hours terminal plasma half-life for Naloxone Hydrochloride half-life window (rapid intravascular/parenteral onset within 15–45 minutes after naloxone hydrochloride administration response profile).

Dosing protocols (1 mg/mL Injection or 4 mg/.25mL Spray or 4 mg/.1mL Spray, Metered or .4 mg/mL Injection, Solution or 4 mg/.1mL Spray) balance efficacy (89.00% patient satisfaction across 11 logs) against nausea (8.4% of reports), headache (7% of reports), dizziness (6.1% of reports), with strict avoidance in severe drug-induced hypersensitivity to naloxone hydrochloride or inactive tablet/vehicle binders; prohibited co-prescriptions or physiological states specified in fda spl 2026-03-19 for naloxone hydrochloride.

Key Monitored Interactions:
CNS Depressants, Sedatives & Opioids Major

Patients taking Naloxone Hydrochloride should avoid unmonitored CNS depressants due to compounded vigilance impairment.
