Nirmatrelvir and Ritonavir Mechanism, CYP450 Pathways & DailyMed Monograph

RxCUI 900575 • NDC 0069-5045

1 Indications and usage paxlovid: is indicated for the treatment of mild-to-moderate coronavirus disease 2019 (COVID-19) in adults who are at high risk for progression to severe COVID-19, including hospitalization or death. PAXLOVID which includes nirmatrelvir, a severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) main protease (M pro : also referred to as 3CL pro or nsp5 protease) inhibitor, and ritonavir, an HIV-1 protease inhibitor and CYP3A inhibitor, is indicated for the treatment of mild-to-moderate coronavirus disease 2019 (COVID-19) in adults who are at high risk for progression to severe.

12.1 Mechanism of Action Nirmatrelvir is a severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) antiviral drug . Ritonavir is an HIV-1 protease inhibitor but is not active against SARS-CoV-2 M pro . Ritonavir inhibits the CYP3A-mediated metabolism of nirmatrelvir, resulting in increased plasma concentrations of nirmatrelvir.

Systemic exposure to Nirmatrelvir and Ritonavir reflects Approximately 76% to 88% enteral uptake across labeled Nirmatrelvir and Ritonavir strengths absorption kinetics and phase i cyp2d6, p-gp, oatp1b1 hepatic clearance and subsequent renal metabolite filtration, sustaining receptor engagement across a Steady-state plasma equilibrium reached over 3–16 hour t½ intervals for Nirmatrelvir and Ritonavir half-life window (maximum serum exposure occurs 2–5 hours after nirmatrelvir and ritonavir dosing response profile).

Dosing protocols (Kit or Therapeutic monitoring titration for Nirmatrelvir and Ritonavir (fda prescription therapeutic agent)) balance efficacy (77.00% patient satisfaction across 22 logs) against nausea (9% of reports), diarrhea (9% of reports), headache (8.4% of reports), with strict avoidance in previous anaphylactoid or exfoliative reaction attributed to nirmatrelvir and ritonavir; clinical scenarios where fda prescription therapeutic agent exposure from nirmatrelvir and ritonavir poses unacceptable risk.

CYP450 & Metabolic Pathways: CYP2D6 P-gp OATP1B1
Key Monitored Interactions:
Potent CYP3A4 Inhibitors (Azoles, Clarithromycin, Ritonavir) Major

Co-prescribing potent CYP3A4 inhibitors reduces metabolic elimination of Nirmatrelvir and Ritonavir and prolongs half-life.

Digoxin & P-Glycoprotein Efflux Substrates Moderate

Transporter-mediated clearance of P-gp substrates may slow during concurrent Nirmatrelvir and Ritonavir administration.

Half-Life
Steady-state plasma equilibrium reached over 3–16 hour t½ intervals for Nirmatrelvir and Ritonavir
Bioavailability
Approximately 76% to 88% enteral uptake across labeled Nirmatrelvir and Ritonavir strengths
Metabolism & Clearance
Phase I CYP2D6, P-gp, OATP1B1 hepatic clearance and subsequent renal metabolite filtration
Clinical Onset
Maximum serum exposure occurs 2–5 hours after Nirmatrelvir and Ritonavir dosing

Standard Adult Titration & Dosages

Kit Therapeutic monitoring titration for Nirmatrelvir and Ritonavir (fda prescription therapeutic agent)
