12.1 Mechanism of Action Ondansetron is a selective 5-HT 3 receptor antagonist. While its mechanism of action has not been fully characterized, ondansetron is not a dopamine-receptor antagonist. Serotonin receptors of the 5-HT 3 type are present both peripherally on vagal nerve terminals and centrally in the chemoreceptor trigger zone of the area postrema. It is not certain whether ondansetron’s antiemetic action is mediated centrally, peripherally, or in both sites. However, cytotoxic chemotherapy appears to be associated with release of serotonin from the enterochromaffin cells of the small intestine.
Following administration (2 mg/mL Injection, Solution), Ondansetron achieves Complete (~100%) parenteral systemic delivery for Ondansetron (2 mg/mL Injection, Solution) bioavailability and rapid intravascular/parenteral onset within 15–45 minutes after ondansetron administration onset, governed by a Biphasic clearance with 5–18 hr terminal t½ under Ondansetron maintenance terminal half-life through hepatic enzymatic processing and renal clearance proportional to creatinine clearance (ondansetron).
Before initiating Ondansetron at 2 mg/mL Injection, Solution, clinical safety checklists rule out prior anaphylaxis, angioedema, or severe hypersensitivity to ondansetron or ondansetron excipients; co-administration with prohibited interacting drugs or decompensated organ failure per 2025-02-21 ondansetron spl. Logged tolerability patterns center on headache (9.6% of reports), fatigue (8.7% of reports), diarrhea (8.3% of reports).
Avoid overlapping serotonin-3 receptor antagonist prescriptions alongside Ondansetron to prevent cumulative receptor toxicity.
Shifts in intragastric pH can modestly influence dissolution kinetics of oral Ondansetron dosage forms.
