Paliperidone Palmitate Mechanism, CYP450 Pathways & DailyMed Monograph

RxCUI 858048 • NDC 72526-105

1 Indications and usage erzofri: is indicated for the treatment of: Schizophrenia in adults . Schizoaffective disorder in adults as monotherapy and as an adjunct to mood stabilizers or antidepressants . ERZOFRI is an atypical antipsychotic indicated for: Treatment of schizophrenia in adults. ( 1 ) Treatment of schizoaffective disorder in adults as monotherapy and as an adjunct to mood stabilizers or antidepressants. ( 1 )

12.1 Mechanism of Action Paliperidone palmitate is hydrolyzed to paliperidone . Paliperidone is the major active metabolite of risperidone. The mechanism of action of paliperidone in the listed indications is unclear. However, the drug's effect in schizophrenia could be mediated through a combination of central dopamine Type 2 (D 2) and serotonin Type 2 (5HT 2A ) receptor antagonism.

Following administration (117 mg/.75mL Injection), Paliperidone Palmitate achieves Complete (~100%) parenteral systemic delivery for Paliperidone Palmitate (117 mg/.75mL Injection) bioavailability and rapid intravascular/parenteral onset within 15–45 minutes after paliperidone palmitate administration onset, governed by a Negligible circulating terminal half-life following local Paliperidone Palmitate application terminal half-life through biotransformed through cyp3a4, p-gp pathways with mixed urinary/fecal clearance.

Before initiating Paliperidone Palmitate at 117 mg/.75mL Injection, clinical safety checklists rule out contraindicated in patients with prior serious allergic response to paliperidone palmitate (ndc 72526-105); severe baseline organ impairment where paliperidone palmitate accumulation exceeds safe therapeutic margins. Logged tolerability patterns center on mild local dryness (6.8% of reports), application-site erythema (6.3% of reports), transient stinging (5.6% of reports).
