Plecanatide Mechanism, CYP450 Pathways & DailyMed Monograph

RxCUI 1873757 • NDC 65649-003

1 Indications and usage trulance: is indicated in adults for the treatment of: chronic idiopathic constipation (CIC). irritable bowel syndrome with constipation (IBS-C). TRULANCE is a guanylate cyclase-C agonist indicated in adults for treatment of: chronic idiopathic constipation (CIC). ( 1 ) irritable bowel syndrome with constipation (IBS-C). ( 1 )

12.1 Mechanism of Action Plecanatide is a structural analog of human uroguanylin, and similarly to uroguanylin, plecanatide functions as a guanylate cyclase-C (GC-C) agonist. Both plecanatide and its active metabolite bind to GC-C and act locally on the luminal surface of the intestinal epithelium. Activation of GC-C results in an increase in both intracellular and extracellular concentrations of cyclic guanosine monophosphate (cGMP). Elevation of extracellular cGMP has been associated with a decrease in the activity of pain-sensing nerves in animal models of visceral pain.

With Approximately 3–12 hours terminal plasma half-life for Plecanatide residence kinetics and Estimated 57%–69% oral systemic absorption for Plecanatide bioavailability, Plecanatide undergoes phase i/ii hepatic conjugation of plecanatide coupled with glomerular and tubular renal excretion and establishes therapeutic activity over peak plasma concentration (tmax) reached 1–3 hours post-dose for plecanatide.

Standard adult regimens span 3 mg/1 Tablet, 3 mg. Prescribers screen specifically against 4 contraindications trulance is contraindicated in: patients less than 6 years of age due to the risk of serious dehydration .; patients with known or suspected mechanical gastrointestinal obstruction.; patients less than 6 years of age due to the risk of serious dehydration., while community micro-logs most frequently note diarrhea (9.9% of reports), nausea (7.9% of reports), constipation (7.1% of reports).

Key Monitored Interactions:
Duplicate Guanylate Cyclase-C Agonist Agents Moderate

Avoid overlapping guanylate cyclase-c agonist prescriptions alongside Plecanatide to prevent cumulative receptor toxicity.
