Rituximab Mechanism, CYP450 Pathways & DailyMed Monograph

RxCUI 1657862 • NDC 50242-051

1 Indications and usage rituxan: is a CD20-directed cytolytic antibody indicated for the treatment of: Adult patients with Non-Hodgkin's Lymphoma (NHL) ( 1.1 ). Relapsed or refractory, low grade or follicular, CD20-positive B-cell NHL as a single agent. Previously untreated follicular, CD20-positive, B-cell NHL in combination with first line chemotherapy and, in patients achieving a complete or partial response to a rituximab product in combination with chemotherapy, as single-agent maintenance therapy.

12.1 Mechanism of Action Rituximab is a monoclonal antibody that targets the CD20 antigen expressed on the surface of pre-B and mature B-lymphocytes. Upon binding to CD20, rituximab mediates B-cell lysis. Possible mechanisms of cell lysis include complement dependent cytotoxicity (CDC) and antibody dependent cell mediated cytotoxicity (ADCC). B cells are believed to play a role in the pathogenesis of rheumatoid arthritis (RA) and associated chronic synovitis.

With Approximately 3–14 hours terminal plasma half-life for Rituximab residence kinetics and Complete (~100%) parenteral systemic delivery for Rituximab (10 mg/mL Injection, Solution) bioavailability, Rituximab undergoes phase i/ii hepatic conjugation of rituximab coupled with glomerular and tubular renal excretion and establishes therapeutic activity over rapid intravascular/parenteral onset within 15–45 minutes after rituximab administration.

Standard adult regimens span 10 mg/mL Injection, Solution, Organ-function-adjusted Rituximab maintenance interval. Prescribers screen specifically against known hypersensitivity to rituximab or structurally related cd20-directed cytolytic antibody compounds; concomitant contra-labeled cyp/transporter therapies during active rituximab administration, while community micro-logs most frequently note nausea (8.5% of reports), headache (8.2% of reports), dizziness (6.7% of reports).

Key Monitored Interactions:
Duplicate CD20-directed Cytolytic Antibody Agents Moderate

Avoid overlapping cd20-directed cytolytic antibody prescriptions alongside Rituximab to prevent cumulative receptor toxicity.

High-Dose Botanical & Nutraceutical Extracts Minor

Unstandardized herbal extracts may interact pharmacodynamically with Rituximab maintenance therapy.
