Solriamfetol Mechanism, CYP450 Pathways & DailyMed Monograph

RxCUI 2121756 • NDC 81968-350

1 Indications and usage sunosi: is indicated to improve wakefulness in adult patients with excessive daytime sleepiness associated with narcolepsy or obstructive sleep apnea (OSA) . Limitations of Use SUNOSI is not indicated to treat the underlying airway obstruction in OSA. Ensure that the underlying airway obstruction is treated (e.g., with continuous positive airway pressure (CPAP)) for at least one month prior to initiating SUNOSI for excessive daytime sleepiness. Modalities to treat the underlying airway obstruction should be continued during treatment with SUNOSI.

12.1 Mechanism of Action The mechanism of action of solriamfetol to improve wakefulness in patients with excessive daytime sleepiness associated with narcolepsy or obstructive sleep apnea is unclear. However, its efficacy could be mediated through its activity as a dopamine and norepinephrine reuptake inhibitor (DNRI).

With Steady-state plasma equilibrium reached over 6–18 hour t½ intervals for Solriamfetol residence kinetics and ~95% systemic bioavailability (Solriamfetol) bioavailability, Solriamfetol undergoes predominantly hepatic biotransformation with renal elimination of polar solriamfetol conjugates and establishes therapeutic activity over maximum serum exposure occurs 2–5 hours after solriamfetol dosing.

Standard adult regimens span 75 mg/1 Tablet, Film Coated, 150 mg/1 Tablet, Film Coated. Prescribers screen specifically against history of severe allergic sensitivity to solriamfetol active moiety or formulation vehicle; acute clinical instability or interacting polypharmacy excluded under 2026-05-05 solriamfetol labeling, while community micro-logs most frequently note nausea (12.9% of reports), headache (11.5% of reports), insomnia (7.2% of reports).

Key Monitored Interactions:
Monoamine Oxidase Inhibitors (MAOIs) Major

Concomitant MAOI therapy potentiates hazardous biogenic amine accumulation during Solriamfetol treatment.

Overlapping Therapeutic Class Formulations Moderate

Verify medication reconciliation lists so Solriamfetol is not inadvertently duplicated with another dopamine and norepinephrine reuptake inhibitor.
