Tezacaftor and Ivacaftor Mechanism, CYP450 Pathways & DailyMed Monograph

RxCUI 1243046 • NDC 51167-113

1 Indications and usage symdeko: is indicated for the treatment of cystic fibrosis (CF) in patients aged 6 years and older who are homozygous for the F508del mutation or who have at least one mutation in the cystic fibrosis transmembrane conductance regulator ( CFTR ) gene that is responsive to tezacaftor/ivacaftor based on in vitro data and/or clinical evidence . If the patient's genotype is unknown, an FDA-cleared CF mutation test should be used to detect the presence of a CFTR mutation followed by verification with bi-directional sequencing when recommended by the mutation test instructions for use.

12.1 Mechanism of Action Tezacaftor facilitates the cellular processing and trafficking of select mutant forms of CFTR (including F508del-CFTR) to increase the amount of mature CFTR protein delivered to the cell surface. Ivacaftor is a CFTR potentiator that facilitates increased chloride transport by potentiating the channel-open probability (or gating) of the CFTR protein at the cell surface. For ivacaftor to function CFTR protein must be present at the cell surface. Ivacaftor can potentiate the CFTR protein delivered to the cell surface by tezacaftor, leading to a further enhancement of chloride transport than either agent alone.

With Terminal t½ estimated at 10–19 hrs depending on renal/hepatic reserve (Tezacaftor and Ivacaftor) residence kinetics and F ≈ 52%–64% systemic availability after standard oral Tezacaftor and Ivacaftor administration bioavailability, Tezacaftor and Ivacaftor undergoes mixed metabolic conjugation and unchanged urinary excretion across tezacaftor and ivacaftor regimens and establishes therapeutic activity over time to peak concentration spans 1 to 4 hrs under tezacaftor and ivacaftor maintenance.

Standard adult regimens span Kit, 50 mg, 75 mg. Prescribers screen specifically against documented intolerance or hypersensitivity reaction involving tezacaftor and ivacaftor (fda prescription therapeutic agent); concurrent restricted medications flagged in section 4/7 of the tezacaftor and ivacaftor dailymed monograph (1243046), while community micro-logs most frequently note nausea (12.4% of reports), headache (11% of reports), dizziness (6.7% of reports).

Key Monitored Interactions:
Potent CYP3A4 Inhibitors (Azoles, Clarithromycin, Ritonavir) Major

Inhibiting CYP3A4 clearance during Tezacaftor and Ivacaftor therapy increases concentration-dependent adverse effects.

Hepatically Conjugated Concomitant Therapies Moderate

Assess baseline hepatic panels and interacting prescriptions when starting Tezacaftor and Ivacaftor (fda prescription therapeutic agent).
