RxCUI 2740019 • NDC 84976-001

1 Indications and usage avlayah: is indicated for the treatment of neurologic manifestations of Hunter syndrome (Mucopolysaccharidosis type II, MPS II) when initiated in presymptomatic or symptomatic pediatric patients weighing at least 5 kg prior to advanced neurologic impairment. This indication is approved under accelerated approval based on the reduction of cerebrospinal fluid heparan sulfate . Continued approval for this indication may be contingent upon verification and description of clinical benefit in a confirmatory trial(s).

12.1 Mechanism of Action Hunter syndrome is an inherited X-linked recessive lysosomal storage disease caused by a deficiency of iduronate-2-sulfatase (IDS), a lysosomal enzyme, that degrades heparan sulfate (HS) and dermatan sulfate (DS), the two primary glycosominoglycans (GAGs) in the lysosome. Insufficiency or absence of IDS leads to accumulation of GAGs, including HS and DS, and subsequent lysosome dysfunction in multiple organs and tissues, including the central nervous system (CNS). Tividenofusp alfa-eknm provides an exogenous source of IDS.

Systemic exposure to Tividenofusp Alfa-Eknm reflects Complete (~100%) parenteral systemic delivery for Tividenofusp Alfa-Eknm (150 mg/5mL Injection, Powder, Lyophilized, for Solution) absorption kinetics and dual hepatorenal clearance pathway combining glucuronidation and urinary filtration of tividenofusp alfa-eknm, sustaining receptor engagement across a Mean elimination half-life of 7 to 18 hours across adult Tividenofusp Alfa-Eknm dosing half-life window (rapid intravascular/parenteral onset within 15–45 minutes after tividenofusp alfa-eknm administration response profile).

Dosing protocols (150 mg/5mL Injection, Powder, Lyophilized, for Solution or Response-guided Tividenofusp Alfa-Eknm dose modification under follow-up) balance efficacy (75.00% patient satisfaction across 14 logs) against headache (10.7% of reports), diarrhea (9.4% of reports), nasopharyngitis (8.8% of reports), with strict avoidance in history of severe allergic sensitivity to tividenofusp alfa-eknm active moiety or formulation vehicle; acute clinical instability or interacting polypharmacy excluded under 2026-03-20 tividenofusp alfa-eknm labeling.

Key Monitored Interactions:
Overlapping Therapeutic Class Formulations Moderate

Verify medication reconciliation lists so Tividenofusp Alfa-Eknm is not inadvertently duplicated with another hydrolytic lysosomal glycosaminoglycan-specific enzyme.

Gastric pH-Altering Agents (PPIs & H2 Blockers) Minor
