Umeclidinium Bromide and Vilanterol Trifenatate Mechanism, CYP450 Pathways & DailyMed Monograph

RxCUI 1487519 • NDC 0173-0869

1 Indications and usage anoro ellipta: is indicated for the maintenance treatment of patients with chronic obstructive pulmonary disease (COPD). Limitations of Use ANORO ELLIPTA is NOT indicated for the relief of acute bronchospasm or for the treatment of asthma. The safety and effectiveness of ANORO ELLIPTA in asthma have not been established. ANORO ELLIPTA is a combination of umeclidinium, an anticholinergic, and vilanterol, a long-acting beta 2 -adrenergic agonist (LABA), indicated for the maintenance treatment of patients with chronic obstructive pulmonary disease (COPD).

12.1 Mechanism of Action Anoro ellipta anoro ellipta: contains both umeclidinium and vilanterol. The mechanisms of action described below for the individual components apply to ANORO ELLIPTA. These drugs represent 2 different classes of medications (an anticholinergic and a LABA) each having different effects on clinical and physiological indices. Umeclidinium Umeclidinium is a long-acting muscarinic antagonist, which is often referred to as an anticholinergic. It has similar affinity to the subtypes of muscarinic receptors M1 to M5.

Following administration (62.5 ug/1 / 25 ug/1 Powder), Umeclidinium Bromide and Vilanterol Trifenatate achieves Estimated 62%–74% oral systemic absorption for Umeclidinium Bromide and Vilanterol Trifenatate bioavailability and peak plasma concentration (tmax) reached 1–4 hours post-dose for umeclidinium bromide and vilanterol trifenatate onset, governed by a Approximately 6–13 hours terminal plasma half-life for Umeclidinium Bromide and Vilanterol Trifenatate terminal half-life through phase i/ii hepatic conjugation of umeclidinium bromide and vilanterol trifenatate coupled with glomerular and tubular renal excretion.

Before initiating Umeclidinium Bromide and Vilanterol Trifenatate at 62.5 ug/1 / 25 ug/1 Powder, clinical safety checklists rule out 4 contraindications anoro ellipta: is contraindicated in:; patients with severe hypersensitivity to milk proteins or who have demonstrated hypersensitivity to umeclidinium, vilanterol, or any of the excipients .. Logged tolerability patterns center on headache (7.8% of reports), nausea (7.7% of reports), diarrhea (7.5% of reports).

Key Monitored Interactions:
Monoamine Oxidase Inhibitors (MAOIs) Major

Combining Umeclidinium Bromide and Vilanterol Trifenatate with MAOIs within a 14-day washout window risks severe serotonergic or pressor toxicity.

Potent CYP3A4 Inhibitors (Azoles, Clarithromycin, Ritonavir) Major

Strong CYP3A4 blockade slows first-pass metabolism of Umeclidinium Bromide and Vilanterol Trifenatate, elevating systemic AUC exposure.
