Evidence Desk Prompt • Hyperbolic taper: what step sizes and pauses did your prescriber approve?
Hyperbolic tapering means reducing by a fixed percentage of the current dose, because the effect on serotonin transporter occupancy is nonlinear. Published modelling (Horowitz & Taylor, Lancet Psychiatry 2019) suggests roughly 10% of the current dose per step, each lasting about 2-4 weeks, with a pause if symptoms emerge. Example: 50 mg → 45 → 40 → 36 → 32 → 29 → 26 mg. Low doses need the finest cuts, and sertraline 20 mg/mL liquid or compounded capsules allow this, while tablets do not. Discussion questions: 1) What step size and interval did you and your prescriber agree on? 2) Which formulation did you use, and what was the cash price per month? 3) How did you tell withdrawal symptoms (dizziness, 'brain zaps', irritability) from relapse? Please do not share advice to change another person's dose. Share only what your clinician approved.
Horowitz & Taylor (Lancet Psychiatry 2019, PubMed) proposed reductions of about 10% of the current dose with hyperbolic spacing because of nonlinear receptor occupancy. The sertraline DailyMed label advises gradual dose reduction and describes discontinuation symptoms such as dizziness, irritability, and paresthesias.
