What should I expect in the first 4 weeks of sertraline, and when do I call my prescriber?
Common patient question: 'Week 2 and I feel jittery and nauseated, not better. Is this normal?' Early adverse effects (nausea, loose stools, jitteriness, insomnia, sweating) often peak in weeks 1-2 and fade. Benefit usually builds more slowly, with partial change by weeks 2-4 and a fuller read at 6-8 weeks. Label dosing: for MDD, start at 50 mg/day; for panic disorder, PTSD and social anxiety, start at 25 mg/day for 1 week, then 50 mg. Dose changes are made at intervals of at least 1 week, with a range of 50-200 mg. Practical steps: take it with food, keep a daily 0-10 log for mood, anxiety, sleep and nausea, and avoid stacking new supplements in week 1 so side effects stay attributable. Call promptly for worsening mood, agitation, or suicidal thoughts (boxed warning, highest vigilance under age 25), or for fever with tremor and diarrhea. Call 988 in the US for crisis support.
The sertraline FDA DailyMed label recommends a 50 mg/day start for MDD and a 25 mg/day start for 1 week in panic disorder, with dose changes at intervals of at least 1 week. The boxed warning describes increased suicidal thinking in patients up to age 24 during early treatment.
