Dexmedetomidine Hydrochloride Mechanism, CYP450 Pathways & DailyMed Monograph

RxCUI 1718906 • NDC 25021-615

Dexmedetomidine hydrochloride in 0.9% sodium chloride injection is a alpha 2 -adrenergic receptor agonist indicated for: Sedation of initially intubated and mechanically ventilated adult patients during treatment in an intensive care setting. Administer dexmedetomidine hydrochloride in 0.9% sodium chloride injection by continuous infusion not to exceed 24 hours. ( 1.1 ) Sedation of non-intubated adult patients prior to and/or during surgical and other procedures.

12.1 Mechanism of Action Dexmedetomidine is a relatively selective centrally acting alpha 2 -adrenergic agonist with sedative properties. Alpha 2 selectivity is observed in animals following slow intravenous infusion of low and medium doses (10 to 300 mcg/kg). Both alpha 1 and alpha 2 activity is observed following slow intravenous infusion of high doses (≥1,000 mcg/kg) or with rapid intravenous administration.

Systemic exposure to Dexmedetomidine Hydrochloride reflects Complete (~100%) parenteral systemic delivery for Dexmedetomidine Hydrochloride (4 ug/mL Injection, Solution) absorption kinetics and mixed metabolic conjugation and unchanged urinary excretion across dexmedetomidine hydrochloride regimens, sustaining receptor engagement across a 6 minutes terminal elimination (Dexmedetomidine Hydrochloride SPL) half-life window (rapid intravascular/parenteral onset within 15–45 minutes after dexmedetomidine hydrochloride administration response profile).

Dosing protocols (4 ug/mL Injection, Solution or 100 ug/mL Injection, Solution or 200 ug/50mL Injection or 4 ug/mL Injection or 100 ug/mL Injection, Solution, Concentrate) balance efficacy (87.00% patient satisfaction across 7 logs) against nausea (8.4% of reports), dry mouth (8.2% of reports), headache (7% of reports), with strict avoidance in severe drug-induced hypersensitivity to dexmedetomidine hydrochloride or inactive tablet/vehicle binders; prohibited co-prescriptions or physiological states specified in fda spl 2023-10-06 for dexmedetomidine hydrochloride.

Key Monitored Interactions:
CNS Depressants, Sedatives & Opioids Major

Patients taking Dexmedetomidine Hydrochloride should avoid unmonitored CNS depressants due to compounded vigilance impairment.
