Gallium Citrate Ga-67 Mechanism, CYP450 Pathways & DailyMed Monograph

RxCUI RXN-69945180-1786 • NDC 69945-180

Indications and usage: Gallium Citrate Ga 67 Injection may be useful to demonstrate the presence and extent of Hodgkin's disease, lymphoma, and bronchogenic carcinoma. Positive gallium Ga-67 uptake in the absence of prior symptoms warrants follow-up as an indication of a potential disease state. Gallium Citrate Ga 67 Injection may be useful as an aid in detecting some acute inflammatory lesions.

CLINICAL PHARMACOLOGY Gallium Citrate Ga 67, with no carrier added, has been found to concentrate in certain viable primary and metastatic tumors as well as focal sites of infection. The mechanism of concentration is unknown, but investigational studies have shown that gallium Ga-67 accumulates in lysosomes and is bound to a soluble intracellular protein. It has been reported in the scientific literature that following intravenous injection, the highest tissue concentration of gallium Ga-67 - other than tumors and sites of infection - is the renal cortex.

Systemic exposure to Gallium Citrate Ga-67 reflects Complete (~100%) parenteral systemic delivery for Gallium Citrate Ga-67 (2 mCi/mL Injection, Solution) absorption kinetics and predominantly hepatic biotransformation with renal elimination of polar gallium citrate ga-67 conjugates, sustaining receptor engagement across a Steady-state plasma equilibrium reached over 9–21 hour t½ intervals for Gallium Citrate Ga-67 half-life window (rapid intravascular/parenteral onset within 15–45 minutes after gallium citrate ga-67 administration response profile).

Dosing protocols (2 mCi/mL Injection, Solution or Maintenance adjustment for Gallium Citrate Ga-67 based on hepatic/renal clearance) balance efficacy (89.00% patient satisfaction across 11 logs) against nausea (12.3% of reports), headache (6.5% of reports), dizziness (5.9% of reports), with strict avoidance in confirmed ige-mediated allergy or cutaneous reaction to gallium citrate ga-67 (gallium citrate ga-67) components; uncompensated hepatic or renal insufficiency incompatible with gallium citrate ga-67 (2 mci/ml injection, solution) clearance.
