Mannitol Mechanism, CYP450 Pathways & DailyMed Monograph

RxCUI 206536 • NDC 0338-0353

1 Indications and usage osmitrol: is indicated for: ; The reduction of intracranial pressure and treatment of cerebral edema; ; The reduction of elevated intraocular pressure. OSMITROL is an osmotic diuretic, indicated for the reduction of: ; intracranial pressure and treatment of cerebral edema. ( 1 ) ; elevated intraocular pressure. ( 1 )

12.1 Mechanism of Action Mannitol, when administered intravenously, exerts its osmotic diuretic effect as a solute of relatively small molecular size being largely confined to the extracellular space. Mannitol hinders tubular reabsorption of water and enhances excretion of sodium and chloride by elevating the osmolarity of the glomerular filtrate. This increase in extracellular osmolarity affected by the intravenous administration of mannitol will induce the movement of intracellular water to the extracellular and vascular spaces.

Clinical clearance of Mannitol (Osmotic Diuretic) relies on local esterase/enzymatic cleavage of mannitol with minimal hepatic cytochrome burden with a 0.5 to 2.5 hours terminal elimination (Mannitol SPL) half-life; patients absorbing at Complete (~100%) parenteral systemic delivery for Mannitol (20 g/100mL Injection, Solution) bioavailability typically observe target onset around rapid intravascular/parenteral onset within 15–45 minutes after mannitol administration.

FDA clinical labeling for Mannitol restricts administration in 4 contraindications osmitrol is contraindicated in patients with:; known hypersensitivity to mannitol; severe hypovolemia across 20 g/100mL Injection, Solution, 12.5 g/50mL Injection, Solution, 250 mg/mL Injection, Solution, 10 g/100mL Injection, Solution, Kit, with self-reported patient tracking dominated by nausea (11.8% of reports), headache (10.4% of reports), vomiting (5.4% of reports).

Key Monitored Interactions:
Duplicate Osmotic Diuretic Agents Moderate

Avoid overlapping osmotic diuretic prescriptions alongside Mannitol to prevent cumulative receptor toxicity.

Ethanol & Dietary Bioavailability Modifiers Minor

Alcohol or high-fat meal timing can shift absorption rate and orthostatic tolerance during Mannitol dosing.
