Sacubitril and Valsartan Mechanism, CYP450 Pathways & DailyMed Monograph

RxCUI 1656349 • NDC 71610-982

Sacubitril and valsartan tablets are a combination of sacubitril, a neprilisin inhibitor, and valsartan, an angiotensin II receptor blocker, and sacubitril and valsartan tablets are indicated: ; to reduce the risk of cardiovascular death and hospitalization for heart failure in adult patients with chronic heart failure. Benefits are most clearly evident in patients with left ventricular ejection fraction (LVEF) below normal. ( 1.1 ) ; for the treatment of symptomatic heart failure with systemic left ventricular systolic dysfunction in pediatric patients aged one year and older.

12.1 Mechanism of Action Sacubitril and valsartan tablets contains a neprilysin inhibitor, sacubitril, and an angiotensin receptor blocker, valsartan. Sacubitril and valsartan tablets inhibits neprilysin (neutral endopeptidase; NEP) via LBQ657, the active metabolite of the prodrug sacubitril, and blocks the angiotensin II type-1 (AT 1 ) receptor via valsartan.

Systemic exposure to Sacubitril and Valsartan reflects Systemic oral bioavailability of roughly 62%–74% following first-pass transit absorption kinetics and hepatic enzymatic processing and renal clearance proportional to creatinine clearance (sacubitril and valsartan), sustaining receptor engagement across a Biphasic clearance with 8–17 hr terminal t½ under Sacubitril and Valsartan maintenance half-life window (initial pharmacodynamic onset within 1–4 hrs (peak cmax) for sacubitril and valsartan response profile).

Dosing protocols (49 mg/1 / 51 mg/1 Tablet, Film Coated or 97 mg/1 / 103 mg/1 Tablet, Film Coated or 24 mg/1 / 26 mg/1 Tablet, Film Coated or 24 mg/1 / 26 mg/1 Tablet or 49 mg/1 / 51 mg/1 Tablet) balance efficacy (91.00% patient satisfaction across 15 logs) against headache (7.3% of reports), dizziness (6.5% of reports), fatigue (5.2% of reports), with strict avoidance in sacubitril and valsartan tablets are contraindicated:; in patients with hypersensitivity to any component; in patients with a history of angioedema related to previous ace inhibitor or arb therapy.

Key Monitored Interactions:
RAAS Modulators & Potassium-Sparing Diuretics Moderate

Combining Sacubitril and Valsartan with ACE inhibitors, ARBs, or aldosterone blockers can shift serum potassium and eGFR.

Duplicate Angiotensin 2 Receptor Blocker Agents Moderate

Avoid overlapping angiotensin 2 receptor blocker prescriptions alongside Sacubitril and Valsartan to prevent cumulative receptor toxicity.

Half-Life
