1 Indications and usage lokelma: is indicated for the treatment of hyperkalemia in adults. Limitation of Use LOKELMA should not be used as an emergency treatment for life-threatening hyperkalemia because of its delayed onset of action . LOKELMA is a potassium binder indicated for the treatment of hyperkalemia in adults. 12.1 Mechanism of Action LOKELMA (sodium zirconium cyclosilicate) is a non-absorbed zirconium silicate that preferentially captures potassium in exchange for hydrogen and sodium. In vitro , LOKELMA has a high affinity for potassium ions, even in the presence of other cations such as calcium and magnesium. LOKELMA increases fecal potassium excretion through binding of potassium in the lumen of the gastrointestinal tract. Binding of potassium reduces the concentration of free potassium in the gastrointestinal lumen, thereby lowering serum potassium levels.
Pharmacokinetic profiling of Sodium Zirconium Cyclosilicate demonstrates an elimination half-life of Terminal t½ estimated at 4–10 hrs depending on renal/hepatic reserve (Sodium Zirconium Cyclosilicate) with F ≈ 79%–91% systemic availability after standard oral Sodium Zirconium Cyclosilicate administration systemic bioavailability; clearance proceeds via mixed metabolic conjugation and unchanged urinary excretion across sodium zirconium cyclosilicate regimens, reaching clinical onset within time to peak concentration spans 1 to 3 hrs under sodium zirconium cyclosilicate maintenance.
Titration schedules for Sodium Zirconium Cyclosilicate (5 g/5g Powder, for Suspension → 10 g/10g Powder, for Suspension) require excluding documented intolerance or hypersensitivity reaction involving sodium zirconium cyclosilicate (fda prescription therapeutic agent); concurrent restricted medications flagged in section 4/7 of the sodium zirconium cyclosilicate dailymed monograph (2047642); real-world patient logs highlight nausea (7.8% of reports), headache (6.7% of reports), dizziness (6% of reports) alongside a 87.00% Worth-It consensus.
Check serum potassium and creatinine periodically if Sodium Zirconium Cyclosilicate is co-administered with potassium-retaining agents.
Divalent and trivalent cations bind Sodium Zirconium Cyclosilicate intraluminally and blunt peak oral plasma concentrations.
