Female • Age 45-59 • Week 12 Protocol Check: iron, psyllium, magnesium timing and constipation on tirzepatide
Constipation is one of the most common GI effects with tirzepatide, and the fixes can collide. Psyllium husk (5 g in 8 oz of water, 1-2x daily) can slow absorption of other drugs, so many pharmacists advise taking other medicines at least 2 hours apart. Iron bisglycinate and magnesium glycinate are both divalent minerals, so splitting them by about 2 hours may help absorption. Ferritin below 30 ng/mL suggests iron deficiency, and metformin users should check B12 (below 200 pg/mL is deficient, 200-300 is borderline). Questions for the group: 1) What schedule worked, for example psyllium at lunch, iron at breakfast, magnesium at bedtime? 2) Did you check ferritin or B12 before supplementing? 3) How many weeks until bowel habits settled? 4) What fluid target did you use?
Psyllium product labeling advises separating other medications by about 2 hours, and NIH ODS notes iron absorption is reduced by competing minerals and that a ferritin below 30 ng/mL indicates depleted stores. Metformin labeling warns of decreased vitamin B12 levels, with deficiency commonly defined as below 200 pg/mL.
