Tirzepatide 7.5 mg + whey 3x/day + lifting 3x/wk (24 wks update): Age 45-59 male, starting weight 118 kg, with prediabetes, sleep apnea and high ApoB — what worked for others here?

Age 45-59 male, starting weight 118 kg, with prediabetes, sleep apnea and high ApoB. My main change was protein-first meals. I set a target of 140 g/day, about 1.2 g/kg, using whey isolate 25 g at breakfast and mid-afternoon plus a real-food dinner. I did full-body resistance training three days a week and added creatine 5 g daily. I lost 17 kg over 24 weeks. My waist dropped 11 cm, and grip strength stayed the same. Constipation eased after adding psyllium 5 g at lunch. Labs at week 24 showed ApoB down from 118 to 86 mg/dL, and my sleep apnea device pressure was reduced after a repeat sleep study. Everything was done with my clinician.

SURMOUNT-OSA (NEJM 2024) showed tirzepatide reduced AHI by up to about 29 events/hour in adults with obesity and moderate-to-severe OSA, and Zepbound is FDA-approved for that indication. ApoB below 90 mg/dL is a common risk-based treatment target, and the 2018 ACC/AHA cholesterol guideline treats ApoB of 130 mg/dL or higher as a risk-enhancing factor.

Helpful structured report. Please keep your pressure settings under your sleep clinician's supervision, because weight loss can change CPAP requirements. ApoB is more informative than LDL-C when triglycerides are high, so tracking it alongside your weight changes is reasonable.

Same here. Lifting 3 days a week mattered more than I expected. Early on I was weak and light-headed, which improved once I stopped skipping breakfast protein and drank more water.
