How do tirzepatide and semaglutide titration schedules differ, and is it okay to stay on a dose longer than 4 weeks?
Both drugs start low to limit GI effects. Tirzepatide (Zepbound/Mounjaro) starts at 2.5 mg weekly for 4 weeks, then 5 mg. After that, the dose rises in 2.5 mg steps no sooner than every 4 weeks, to a maximum of 15 mg. Wegovy (semaglutide) goes 0.25 mg, 0.5 mg, 1 mg, 1.7 mg, then 2.4 mg, with 4 weeks at each step and 16 weeks to reach maintenance. The labels set a minimum time per step, not a maximum. If nausea, early fullness, or reduced protein intake is a problem, prescribers often hold the current dose another 4 weeks. Tell your prescriber about vomiting, severe abdominal pain, or dehydration, and don't skip ahead on your own. Tirzepatide can reduce oral contraceptive exposure, so the label advises a non-oral or barrier method for 4 weeks after starting and after each dose increase.
FDA DailyMed labeling for Zepbound lists a 2.5 mg start, 2.5 mg increments at least 4 weeks apart, and a 15 mg maximum. Wegovy labeling lists 0.25 mg escalating every 4 weeks to a 2.4 mg maintenance dose, and in SURMOUNT-1 (NEJM 2022) 15 mg tirzepatide produced a mean 20.9% weight loss at 72 weeks.
