Lisinopril 20 mg to Losartan 50 mg, BMP at week 2 (10 wks update): Male, 52, hypertension — what worked for others here?
Male, 52, hypertension. I had a dry cough on lisinopril 20 mg for about 4 months, and it got worse when lying down. My doctor stopped lisinopril and started losartan 50 mg the next morning, taken at the same time daily. Changes I tracked: week 1, cough unchanged; week 3, about 60% better; week 6, gone. My BMP at week 2 showed potassium 4.4 mEq/L and creatinine 1.0 mg/dL, matching baseline. My home BP average (validated upper-arm cuff, 2 readings, 7 days) went from 136/86 on lisinopril to 129/82 on losartan. I did not add potassium supplements or salt substitute. If you are on losartan, bring a symptom log and lab dates to your follow-up. Cough persisting beyond 3 months should prompt a review for other causes such as reflux, asthma, or lung issues.
ACE inhibitor cough is a class effect from bradykinin accumulation, and ARB cough rates are close to placebo (FDA DailyMed losartan label; PubMed meta-analyses of ARB tolerability). The losartan label advises monitoring serum potassium and creatinine after starting therapy, particularly in renal impairment.
