Sodium Phosphates Mechanism, CYP450 Pathways & DailyMed Monograph

RxCUI 1872384 • NDC 65145-200

INDICATIONS & USAGE Sodium phosphates injection, 3 mM P/mL is indicated as a source of phosphorus, for addition to large volume intravenous fluids, to prevent or correct hypophosphatemia in patients with restricted or no oral intake. It is also useful as an additive for preparing specific parenteral fluid formulas when the needs of the patient cannot be met by standard electrolyte or nutrient solutions. The concomitant amount of sodium (Na + 4 mEq/mL) must be calculated into total electrolyte dose of such prepared solutions.

CLINICAL PHARMACOLOGY Phosphorus in the form of organic and inorganic phosphate has a variety of important biochemical functions in the body and is involved in many significant metabolic and enzyme reactions in almost all organs and tissues. It exerts a modifying influence on the steady state of calcium levels, a buffering effect on acid-base equilibrium and a primary role in the renal excretion of hydrogen ion. Phosphorus is present in plasma and other extracellular fluid, in cell membranes and intracellular fluid, as well as in collagen and bone tissues.

Pharmacokinetic profiling of Sodium Phosphates demonstrates an elimination half-life of Steady-state plasma equilibrium reached over 9–19 hour t½ intervals for Sodium Phosphates with Complete (~100%) parenteral systemic delivery for Sodium Phosphates (276 mg/mL / 268 mg/mL Injection, Solution) systemic bioavailability; clearance proceeds via predominantly hepatic biotransformation with renal elimination of polar sodium phosphates conjugates, reaching clinical onset within rapid intravascular/parenteral onset within 15–45 minutes after sodium phosphates administration.

Titration schedules for Sodium Phosphates (276 mg/mL / 268 mg/mL Injection, Solution → Maintenance adjustment for Sodium Phosphates based on hepatic/renal clearance) require excluding contraindications sodium phosphate is contraindicated in diseases where high phosphorus or low calcium levels may be encountered, and in patients with hypernatremia.; confirmed ige-mediated allergy or cutaneous reaction to sodium phosphates (sodium phosphates) components; real-world patient logs highlight nausea (7.9% of reports), headache (7.9% of reports), fatigue (5.6% of reports) alongside a 89.00% Worth-It consensus.

Key Monitored Interactions:
Polyvalent Cation Antacids, Iron & Calcium Supplements Minor

Separate Sodium Phosphates ingestion from mineral supplements or antacids to preserve gastrointestinal bioavailability.

Renally Cleared Narrow-Index Co-Medications Moderate

Reconcile eGFR-adjusted medications before escalating Sodium Phosphates in polypharmacy regimens.
