Does morning coffee really block levothyroxine absorption? How long should I wait?

I take 100mcg levothyroxine every morning at 6am on an empty stomach. I usually drink coffee around 6:30am. My endocrinologist said to wait 30-60 minutes, but I've read conflicting advice online ranging from 15 minutes to 4 hours. My last TSH was 3.8 mIU/L (range 0.4-4.0), which feels high for me—I'm still fatigued and gaining weight despite being 'in range.' Could the coffee timing be interfering? I also take 200mcg selenium and 25mg iron bisglycinate later in the day. Should I be spacing those differently too?

FDA DailyMed labels for levothyroxine specify 30-60 minute separation from food/beverages for optimal absorption; coffee's polyphenols and calcium (if added) can chelate thyroid hormone. NIH studies show iron (any form) requires 4-hour separation, selenium 2-hour minimum to avoid competitive binding at intestinal transporters.

The FDA label recommends 30-60 minutes before food or beverages other than water. Coffee's chlorogenic acids can reduce levothyroxine absorption by 27-36% in pharmacokinetic studies. Your TSH at 3.8 mIU/L suggests suboptimal dosing or absorption interference. Try waiting 60 minutes before coffee, and confirm your iron is taken at least 4 hours apart (午後 or bedtime). Recheck TSH in 6-8 weeks.

I had the exact same TSH creep (3.2 to 4.1) until I moved my coffee to 90 minutes post-dose and switched iron to dinner. TSH dropped to 1.8 in 8 weeks. Also make sure you're not taking calcium citrate or vitamin D3 within 4 hours—those blocked me too.
