Anyone else switch Levothyroxine from morning to bedtime so you don't have to wait 60 minutes for coffee? Plus the 72-hour Biotin lab trap!

For two years my TSH bounced between 3.8 and 5.4 mIU/L because I was only waiting 15–20 minutes before drinking my morning latte with oat milk, and taking my calcium/iron multivitamin too close to breakfast. After reading the Dutch randomized trial on bedtime levothyroxine dosing, my endocrinologist let me switch to taking my 88 mcg tablet at 10:30 PM (at least 3 hours after dinner). Eight weeks later my TSH settled at 1.4 mIU/L and my morning brain fog lifted. We also caught a false lab scare last year because my hair/nail supplement had 5,000 mcg of Biotin, which skewed the immunoassay! Do you take your thyroid dose in the morning or at bedtime, and how strict do you have to be with coffee and minerals?

A randomized crossover trial in Archives of Internal Medicine (Bolk et al., PMID 21149749) demonstrated that bedtime levothyroxine intake (3+ hours after the evening meal) reduced TSH by 1.25 mIU/L and increased Free T4 compared to morning intake near coffee. Always pause Biotin (Vitamin B7) for 72 hours before thyroid blood draws.

Bedtime dosing saved my mornings! We stop eating by 7:00 PM and I take my pill at 10:30 PM on a completely empty stomach. My TSH went from 4.6 down to 1.6 without even changing the microgram dose.

If you stay with morning dosing, the trick that worked for me is keeping a glass of water and my pill on the nightstand. When I wake up at 4:30 or 5:00 AM to use the bathroom, I swallow it then and go back to sleep—by 7:00 AM coffee time, the 60-minute window is long done!

That Biotin lab warning is so important. My primary care doctor almost cut my thyroid dose in half because a B-complex with biotin made my Free T4 look falsely high and my TSH look suppressed. Pausing biotin for 4 days showed my true numbers.
