[What Worked For Me • Rating: 4.5/5 • Duration: 16 wks • Protocol: 500mg IR→1000mg ER + B12 + Magnesium]

Started metformin 500mg IR BID for type 2 diabetes (baseline A1C 7.3%, fasting glucose 152 mg/dL). Week 1–2: severe nausea, loose stools. Switched to 1000mg ER with dinner at week 3—nausea resolved by day 5. Added B12 sublingual 1000 mcg daily (baseline B12 was 310 pg/mL, low-normal) and magnesium glycinate 400mg at lunch (separated from ER dose by 2 hours). By week 8, fasting glucose 118 mg/dL; week 16 A1C 6.4%. No GI issues after week 4. Took ER with small protein snack (Greek yogurt) to further reduce nausea. Also on atorvastatin 40mg and bupropion XL 300mg—no interactions.

ER formulations reduce GI side effects by 40–60% vs. IR (PubMed 28456891). B12 supplementation at baseline (even if low-normal) prevents deficiency progression; magnesium glycinate is stone-protective and aids glucose metabolism.

Excellent protocol adherence. ER + food + B12 + magnesium separation is textbook. Recommend repeat B12 labs at week 24 and annually thereafter. Fasting glucose trajectory (152→118 mg/dL) suggests good metformin response; monitor for dose plateau around week 12–16.

This is exactly my path. IR was unbearable; ER + evening meal changed everything. I also added alpha-lipoic acid 300mg BID at week 8 for additional insulin sensitivity. A1C now 6.2% at week 20. B12 labs at week 12 showed stable 340 pg/mL. Highly recommend this protocol.
