[What Worked For Me • Rating: 4/5 • Duration: 24 wks • Protocol: Metformin 1500mg ER + Empagliflozin + B12 + Boswellia]
Type 2 diabetes (baseline A1C 7.6%, fasting glucose 158 mg/dL). Started metformin 1000mg ER week 1; fasting glucose plateaued at 125–135 mg/dL by week 10 despite good adherence. Added empagliflozin 10mg daily at week 8 (eGFR 68 mL/min/1.73m²). By week 12, fasting glucose 108–115 mg/dL. Escalated metformin to 1500mg ER at week 12 (split: 500mg with breakfast, 1000mg with dinner). Week 24 A1C 6.1%. Also taking B12 sublingual 1000 mcg daily (baseline 295 pg/mL), magnesium glycinate 400mg, and boswellia serrata extract 30% AKBA 500mg BID for joint inflammation (comorbid). No GI issues after week 2. Empagliflozin required baseline urine ketone check (negative).
Dual-agent therapy (metformin + SGLT2i) reduces A1C by 1.8–2.2% vs. monotherapy; empagliflozin is particularly effective for dawn phenomenon (PubMed 26356313). Boswellia does not interact with metformin or empagliflozin.
