How many days a month were you taking Sumatriptan or Rizatriptan before realizing it was causing morning rebound headaches—and what preventive broke the cycle?

Last year my migraines crept up from 3 days a month to almost 12 days a month. Because Sumatriptan 50 mg worked so reliably within 90 minutes, I kept taking it whenever I woke up with temple throbbing—not realizing that crossing the 10-treatment-days-per-month threshold triggers Medication Overuse Headache (rebound headaches) as the triptan wears off at 4 AM! My headache neurologist helped me break the cycle by starting a daily preventive foundation: Riboflavin (Vitamin B2) 400 mg with breakfast, Magnesium Glycinate 300 mg at night, CoQ10 150 mg, plus low-dose Propranolol, and strictly capping triptan days at ≤6 days/month (counting treatment DAYS, not pills). By month 3 my migraine days dropped from 12/month down to 2/month! What preventive stack finally lowered your monthly migraine days?

The International Classification of Headache Disorders (ICHD-3) defines triptan-induced Medication Overuse Headache (MOH) as use on ≥10 days/month for >3 months (or ≥15 days/month for simple analgesics). Note:FDA DailyMed labeling for Rizatriptan (Maxalt) requires reducing the dose to 5 mg (max 15 mg/24h) if co-administered with Propranolol, which increases rizatriptan AUC by 70%.

@BlueRidgeMom3 • 30–44F • Riboflavin 400 mg + Magnesium • Week 20

The Riboflavin 400 mg + Magnesium 300 mg combo took a full 8–10 weeks to build up in mitochondrial tissue, so don't quit at week 3! Once I hit week 10, my menstrual migraines went from 4 days down to a mild 1-day headache.

So glad the pinned note mentioned the Propranolol + Rizatriptan interaction! My pharmacist caught that Propranolol slows down the enzyme that clears Rizatriptan, so switching to the 5 mg wafer prevented the heavy chest/neck pressure I used to get on 10 mg.

Tracking 'medication days' on a fridge calendar—where taking Excedrin on Monday and Sumatriptan on Wednesday counts as 2 acute days toward the 10-day limit—was the wake-up call that got me out of rebound.

How many days a month can I take sumatriptan or rizatriptan before rebound headaches start?

The ICHD-3 threshold for medication-overuse headache is triptan use on 10 or more days per month for more than 3 months. For simple analgesics such as acetaminophen or NSAIDs, the threshold is 15 or more days per month. Combination analgesics count at 10 days. Counting days matters more than counting tablets. Two doses on one day is still one day. Daily maximums are 200 mg/day for oral sumatriptan and 30 mg/day for rizatriptan. Rizatriptan is limited to 15 mg/day (5 mg per dose) if you also take propranolol. If you are near 8-9 triptan days a month, that is a signal to talk about preventives, not to wait for rebound. Options include riboflavin, topiramate, propranolol, or a CGRP-targeted drug. Log every acute-medication day so the pattern is visible.

ICHD-3 defines triptan-related medication-overuse headache as 10 or more treatment days/month for over 3 months. FDA DailyMed labeling for Maxalt advises a 5 mg dose (max 15 mg/24 h) with propranolol.

Both triptan labels also warn about serotonin syndrome, especially with other serotonergic drugs, and about cardiovascular contraindications. Do not redose sooner than the label says. For sumatriptan tablets, a second dose is allowed at 2 hours or later, and the 200 mg/day maximum includes all doses.

I hit 11 triptan days a month for a while and my headaches got more frequent. A calendar app that only counted days with any triptan was the wake-up call. After adding a preventive and getting down to about 6 days, the pattern improved.
