For those who got calf or shoulder stiffness on high-dose Atorvastatin (40–80 mg): did switching to low-dose Rosuvastatin + Ezetimibe or adding Ubiquinol (CoQ10) fix it?

My coronary calcium (CAC) score came back at 142, so my preventive cardiologist wanted my ApoB under 65 mg/dL. When we started Atorvastatin 40 mg, my LDL and ApoB plummeted, but after 6 weeks my calves and hamstrings felt heavy and sore after every hike. Instead of giving up on statins, my cardiologist used the RACING trial strategy: we switched to low-dose Rosuvastatin 5 mg (which is hydrophilic rather than lipophilic) paired with Ezetimibe 10 mg, and I added 200 mg Ubiquinol (CoQ10) with dinner. My muscle soreness vanished within two weeks, and my 12-week repeat labs showed my ApoB at 58 mg/dL and LDL at 54 mg/dL—even lower than on high-dose monotherapy! What statin regimen or CoQ10 form worked best for your muscles and labs?

The RACING randomized trial (The Lancet 2022, n=3,780) demonstrated that moderate/low-intensity statin plus ezetimibe 10 mg achieved higher rates of LDL-C <70 mg/dL with significantly lower drug discontinuation for muscle intolerance than high-intensity statin monotherapy.

Adding Ubiquinol 200 mg (make sure the label says Kaneka Ubiquinol in a softgel with oil, not dry ubiquinone powder) took my leg soreness on Atorvastatin 20 mg from a 6/10 down to zero within 3 weeks.

@HarborSwimmer_ME • 60+F • Rosuvastatin 5 mg 3x/wk + Ezetimibe • Week 32

Because Rosuvastatin has a 19-hour half-life, my lipidologist has me take 5 mg Monday/Wednesday/Friday plus daily Ezetimibe 10 mg. My LDL dropped from 162 to 68 mg/dL with zero muscle aches.

Also ask your doctor to check your Vitamin D3 and TSH if you get statin muscle aches! When my 25(OH)D was 18 ng/mL, every statin made my legs ache; once I got Vitamin D up to 44 ng/mL, I tolerated Rosuvastatin 10 mg easily.

ApoB vs LDL-C: which number should I follow on atorvastatin 40 mg plus ezetimibe 10 mg with coronary artery disease?

Public question: "My LDL-C is 62 mg/dL on atorvastatin 40 mg plus ezetimibe 10 mg, but my ApoB is 84 mg/dL. Which number matters?" Evidence Desk answer: LDL-C measures cholesterol mass, while ApoB counts atherogenic particles (one per VLDL, IDL, LDL and Lp(a) particle). They can disagree when triglycerides are high, or with insulin resistance or metabolic syndrome. The 2019 ESC/EAS guidelines give ApoB goals of under 65 mg/dL for very-high-risk patients and under 80 mg/dL for high-risk ones. ACC/AHA guidance lists ApoB of 130 mg/dL or higher as a risk enhancer. With established coronary disease, ApoB of 84 is above the ESC very-high-risk goal even though LDL-C looks good. Discuss options with your prescriber: higher statin intensity, switching to rosuvastatin, or adding a PCSK9 inhibitor. Ask for fasting triglycerides and a one-time Lp(a) as well.

The 2019 ESC/EAS dyslipidaemia guideline sets ApoB targets of <65 mg/dL (very high risk) and <80 mg/dL (high risk), and the 2018 ACC/AHA cholesterol guideline lists ApoB ≥130 mg/dL as a risk-enhancing factor. In IMPROVE-IT (NEJM 2015), adding ezetimibe 10 mg lowered LDL-C to 53 vs 70 mg/dL and cut major events by 2.0 percentage points over 7 years.

One lab caveat: calculated LDL-C using the Friedewald equation becomes unreliable when triglycerides exceed 400 mg/dL and underestimates LDL at low LDL-C values. Ask whether your lab uses the Martin-Hopkins or Sampson equation, or a direct LDL assay. ApoB is not affected by that calculation issue and does not require fasting.

My LDL-C was 58 but ApoB was 91 with triglycerides around 240. Tightening carbs, losing 8 lb, and moving from atorvastatin 20 to 40 mg brought ApoB to 72 at the next draw. I now ask for ApoB at every recheck.

Atorvastatin 40 → Rosuvastatin 5 mg + ezetimibe 10 mg (14 wks update): Background: Male, 52, coronary artery disease, on atorvastatin 40 mg with persistent thigh... — what worked for others here?

Background: Male, 52, coronary artery disease, on atorvastatin 40 mg with persistent thigh aching (about 5/10) and LDL-C of 71 mg/dL. Protocol: With my cardiologist, I had CK (normal), TSH, vitamin D (24 ng/mL) and ALT checked. I stopped atorvastatin for 3 weeks, and the aching dropped to 1/10. I then started rosuvastatin 5 mg plus ezetimibe 10 mg daily, took vitamin D3 2,000 IU, and stopped a red yeast rice capsule I had been taking on the side. Result at week 14: aching about 1-2/10, LDL-C 58 mg/dL, ApoB 69 mg/dL, ALT normal. Takeaway: the combination gave a larger LDL drop than high-dose single therapy without the muscle burden for me. It is one person's experience and not a recommendation.

FDA DailyMed labeling for ezetimibe (Zetia) reports about an additional 21-25% LDL-C lowering when added to a statin, and the Crestor label lists 5 mg as the lowest approved dose. The 2022 ACC Expert Consensus Pathway supports adding non-statin therapy when LDL-C remains above goal on tolerated statin therapy.

Good that you stopped red yeast rice. Its monacolin K is chemically identical to lovastatin, so taking it with a statin stacks statin exposure without a tested dose. Product content also varies widely, and some contain citrinin, a kidney toxin, so label claims are unreliable.

Similar path for me: atorvastatin 40 to rosuvastatin 5 plus ezetimibe 10. LDL-C went from 78 to 55. Before blaming the statin, my doctor also repeated a CK and checked my thyroid, which saved me some worry.

Atorvastatin 40 mg gives me thigh and calf aches: will rosuvastatin be easier on muscles, and what do I check first?

Public question: "Three weeks after starting atorvastatin 40 mg I have dull thigh and calf aching. Will switching to rosuvastatin fix it?" Evidence Desk answer: Switching is a reasonable step, but it is not guaranteed. Rosuvastatin is hydrophilic and barely uses CYP3A4, while atorvastatin is lipophilic and CYP3A4-dependent. Some people tolerate one better, though head-to-head proof of fewer muscle symptoms is limited. First, rule out other causes: check CK, TSH, vitamin D, creatinine and ALT. Ask about new exercise, alcohol, and interacting drugs. Then try a washout of 2-4 weeks. If symptoms resolve and return on rechallenge, a statin is likely involved. A rough equivalence is atorvastatin 40 mg to rosuvastatin 10 mg, so starting at 5-10 mg is common. Stop and call your prescriber urgently for severe pain, weakness, or dark urine.

FDA DailyMed labeling for atorvastatin (Lipitor) and rosuvastatin (Crestor) advises evaluating unexplained muscle pain, tenderness or weakness with CK and discontinuing if CK exceeds 10x ULN or myopathy is suspected. The Cholesterol Treatment Trialists' Collaboration (Lancet 2022) found that statins accounted for only about 1 in 15 muscle symptom reports, mostly in year one.

Add one label point: rosuvastatin exposure is roughly doubled in patients of Asian ancestry, so the label suggests starting at 5 mg. Severe renal impairment (CrCl under 30 mL/min, not on dialysis) caps it at 10 mg. Bring your full medication and supplement list, including red yeast rice, because it contains monacolin K, which is chemically lovastatin.

My CK was normal and TSH was borderline high. After thyroid treatment and a 3-week statin break, I restarted rosuvastatin 5 mg and the aches mostly stopped. Checking labs before blaming the statin saved me a lot of guessing.
