Anyone else get 'balloon ankles' by 5 PM after bumping Amlodipine from 5 mg to 10 mg? Did dropping back to 5 mg + adding an ARB/ACEi fix it?

On Amlodipine 5 mg I had zero side effects, so when my systolic BP was still hovering around 136 mmHg, my doctor doubled my Amlodipine to 10 mg. Within two weeks my socks were leaving deep indentations around my ankles every evening! My doctor explained that dihydropyridine calcium channel blockers dilate the little arteries entering the capillary bed without dilating the veins exiting it, creating hydrostatic pressure that pushes fluid into the ankles. Instead of giving me a water pill (which doesn't fix capillary pressure), she dropped my Amlodipine back to 5 mg and added Losartan 50 mg—because ARBs and ACE inhibitors dilate the venous return side! Within 10 days my ankle swelling disappeared AND my blood pressure dropped to 118/72. What combination worked best for your blood pressure without ankle edema?

In clinical hypertension trials (ACCOMPLISH), Amlodipine peripheral edema is strongly dose-dependent (~2%–3% at 5 mg vs. 10%–15% at 10 mg). Combining Amlodipine 5 mg with an ACE inhibitor or ARB causes post-capillary venular dilation, reducing ankle edema by >50% while providing superior 24-hour BP reduction.

Yes! That exact physiologic explanation (arteriolar vs. venular dilation) is why Lasix didn't help my mom's Amlodipine ankles, but cutting Amlodipine to 5 mg and pairing an ARB cleared them right up.

Taking my 5 mg Amlodipine in the evening rather than first thing in the morning before standing on my feet all day also reduced mild afternoon puffiness for me.

Amlodipine 10 mg to 5 mg + Losartan 50 mg, AM dosing (12 wks update): Female, 63, hypertension with early chronic kidney disease (eGFR 62) — what worked for others here?

Female, 63, hypertension with early chronic kidney disease (eGFR 62). On amlodipine 10 mg, I had pitting ankle swelling by evening, and my shoes stopped fitting. My prescriber lowered amlodipine to 5 mg each morning and added losartan 50 mg for BP and kidney protection. Objective changes: ankle circumference fell from 24.5 cm to 22.5 cm over 4 weeks. Home BP (validated cuff, 7-day average) went from 138/84 to 128/78. A BMP at week 2 showed potassium 4.6 mEq/L, creatinine up slightly from 0.95 to 1.05 mg/dL, which my doctor considered acceptable. I elevated my legs 20 minutes each evening and wore 15-20 mmHg compression stockings during the day. I skipped potassium supplements. Swelling did not vanish entirely, but it became manageable, so I rate it 4 out of 5.

The FDA DailyMed Norvasc label shows peripheral edema of 3.0% at 5 mg versus 10.8% at 10 mg, and adding an ARB or ACE inhibitor lowers edema incidence in published analyses. The losartan label notes that creatinine rises under about 30% from baseline after starting are often tolerated, but larger rises warrant evaluation.

Helpful detail on measuring ankle circumference. Remember that a creatinine rise of up to roughly 30% after starting an ARB is often acceptable, but only your prescriber should judge that. If you add HCTZ later, recheck both sodium and potassium within 1-2 weeks, since thiazides can shift both.

I had a similar experience with amlodipine 10 mg to 5 mg plus losartan. Taking amlodipine in the morning made evening swelling noticeably lighter. Compression socks helped, but I only put them on after confirming with my doctor that my leg circulation was fine.

Amlodipine 10 mg ankle swelling every evening: is it dose-related, and what actually helps besides stopping it?

Public question: "My ankles swell by evening on amlodipine 10 mg. Is it fluid retention, and should I take a water pill?" Evidence Desk answer: Amlodipine edema comes from precapillary arteriolar dilation that pushes fluid into tissue. It is not true sodium retention, so loop and thiazide diuretics usually work poorly for it. It is dose-related. The label reports edema in about 1.8% at 2.5 mg, 3.0% at 5 mg, and 10.8% at 10 mg, versus 0.6% on placebo. Options to discuss with your prescriber: lower to 5 mg and add an ARB or ACE inhibitor, since combining with these reduces edema; take the dose in the morning; elevate legs for 15-20 minutes in the evening; or use compression stockings of 15-20 mmHg. One-sided swelling, sudden weight gain above 2-3 lb in a day, or breathlessness needs prompt evaluation for heart failure, kidney, or clot causes.

The FDA DailyMed Norvasc label reports dose-related peripheral edema of 1.8% (2.5 mg), 3.0% (5 mg), and 10.8% (10 mg) versus 0.6% with placebo. Combining amlodipine with an ACE inhibitor or ARB has been shown to reduce the edema rate (PubMed-indexed analyses, including the 2012 meta-analysis by Makani et al.).

If you also have heart failure or chronic kidney disease, do not assume the swelling is only from amlodipine. Check daily morning weight and ask for BNP and urine albumin-to-creatinine ratio if swelling persists. Non-dihydropyridine blockers like diltiazem or verapamil are generally avoided in reduced ejection fraction heart failure.

My doctor cut amlodipine from 10 mg to 5 mg and added losartan 50 mg. The swelling dropped noticeably within about 3 weeks. Taking amlodipine in the morning and elevating my legs for 20 minutes after dinner also helped.

Atorvastatin 20 mg evening + ezetimibe 10 mg, ApoB tracked (20 wks update): Background: Female, 57, hyperlipidemia and hypertension on amlodipine 5 mg — what worked for others here?

Background: Female, 57, hyperlipidemia and hypertension on amlodipine 5 mg. Atorvastatin 80 mg caused diffuse achiness and fatigue, with LDL-C of 64 mg/dL and ApoB of 82 mg/dL. Protocol: I lowered atorvastatin to 20 mg and added ezetimibe 10 mg, both with dinner. I took omega-3 only after triglycerides were rechecked, and I kept vitamin D3 at 1,000 IU because my 25-OH D was 31 ng/mL. I walked 30 minutes 5 days a week and added soluble fiber at 10 g daily. Results at week 20: aching 1/10, LDL-C 55 mg/dL, ApoB 66 mg/dL, triglycerides 118 mg/dL, home BP averaging 126/78. Takeaway: tracking ApoB, not LDL-C alone, showed the lower statin dose plus ezetimibe was working. Discuss any change with your prescriber.

⚙ Pinned AI Clinical & Label Audit: dotMD-StoichBot (NIH DSLD v9 & PubMed PK Agent)

FDA DailyMed labeling for atorvastatin notes a half-life of about 14 hours, with active metabolites acting 20-30 hours, so dosing time of day is flexible. In IMPROVE-IT (NEJM 2015), adding ezetimibe to simvastatin produced a 6.4% relative risk reduction in the primary composite endpoint.

On omega-3: prescription icosapent ethyl 4 g/day cut events in REDUCE-IT, but the STRENGTH trial of another omega-3 product at 4 g/day did not. Higher-dose fish oil is also linked to more atrial fibrillation (about 2.2% vs 1.3% in STRENGTH). Confirm triglycerides first, then discuss any dose with your prescriber.

Same pattern: atorvastatin 80 gave me heavy legs. At 20 mg with ezetimibe, my LDL-C and ApoB dropped as much as before. The combination plus home BP logs also gave my doctor better data than a single LDL number.

Home BP cuff reads 12 mmHg higher than the clinic: how do you check calibration and cuff size before changing meds?

Evidence Desk prompt for readers on lisinopril, losartan, amlodipine, HCTZ, or metoprolol. Before anyone adjusts a dose based on home numbers, the cuff should be validated and compared against the clinic device. Checklist to compare notes on: Is the device on validatebp.org or a similar validated list? Did you measure mid-upper-arm circumference? Standard adult cuffs fit about 22-32 cm and large cuffs about 32-42 cm, and a cuff that is too small reads high. Seated 5 minutes, back supported, feet flat, arm at heart level, no caffeine in the prior 30 minutes? Two readings 1 minute apart, morning and evening, for 7 days, discarding day 1? Did you carry the cuff to a clinic visit and compare three side-by-side readings? A mean gap within 5 mmHg is generally acceptable. With atrial fibrillation, irregular rhythm can make readings erratic, so share your average of 3 readings. What gap did you find, and which device do you use?

⚙ Pinned AI Clinical & Label Audit: dotMD-StoichBot (NIH DSLD v9 & PubMed PK Agent)

The 2017 ACC/AHA guideline defines home BP of 130/80 mmHg or higher as hypertension and recommends validated upper-arm devices with at least 2 readings per session over 1 week. Cuff bladder size relative to arm circumference materially changes readings, and undersized cuffs can overestimate systolic pressure by roughly 5-10 mmHg or more.

Wrist and finger monitors are generally not recommended for routine use because position errors are large. If your upper-arm cuff and the clinic device differ by more than 10 mmHg across three paired readings, bring both devices to the visit and ask staff to check cuff fit before any dose is changed.

•
@HighPlainsRancher_WY • Age 45-59 • Week 8

My arm measured 35 cm, so the standard cuff was reading about 10 points high. I switched to a large cuff and my 7-day average dropped from 142/88 to 133/84. My doctor then paired readings against the office device.

Male • Age 45-59 • Week 16 Protocol Check: lipid and hematocrit labs on testosterone cypionate plus atorvastatin and amlodipine?

Prompt for men combining testosterone cypionate (TRT) with a statin and a blood pressure drug such as amlodipine. We want to compare how people time labs and what they track. Please share: (1) your testosterone dose and injection schedule (for example 100 mg weekly vs 50 mg twice weekly), (2) whether you draw trough or mid-cycle, (3) LDL-C, ApoB, HDL and non-HDL before and after 12-16 weeks, (4) hematocrit and PSA trends, and (5) home blood pressure averages. Did your lipid numbers shift, particularly HDL? Did anyone need to change statin dose? Also tell us whether amlodipine caused ankle swelling, and whether you take oral minoxidil for hair loss, since it can cause fluid retention and a faster heart rate. Please do not post prescription sources or advice to skip monitoring.

⚙ Pinned AI Clinical & Label Audit: dotMD-StoichBot (NIH DSLD v9 & PubMed PK Agent)

The FDA labeling for testosterone products warns of blood pressure increases and polycythemia, and the TRAVERSE trial (NEJM 2023, n=5,246) found that testosterone was noninferior to placebo for major cardiac events but showed higher atrial fibrillation and acute kidney injury rates. Atorvastatin labeling lists no dose limit with amlodipine, unlike simvastatin, which is capped at 20 mg.

On 80 mg twice weekly, my hematocrit rose from 46% to 51% by week 12 while ApoB stayed around 70 on atorvastatin 20 mg. I draw trough labs the morning before an injection so numbers compare cleanly each time.

•
@FingerLakesHiker_NY • Verified Cohort Member • Week 15

Useful to record the draw timing, since cypionate levels swing between injections. Many clinicians review hematocrit and escalate if it exceeds 52-54%. Home blood pressure should be taken seated after 5 minutes of rest, 2 readings each morning and evening, then averaged over 7 days before anyone adjusts amlodipine.
